Dear Sir:

On March 27,1991, Mr. Walter Haass was notified by telephone of a 10 CFR Part 21 condition that exists at the Houston Lighting and Power South Texas Power Piant involving standby diesel /0 enerator fuel nozzle tips. The purpose of this letter is to provide written notification and a

, preliminary status report.

The Nuclear Engineering Manager of Cooper Bessemer (C B)

Reciprocating was notified by telephone of an emergency diesel generator failure on or about March 18, 1991. The failure involved Houston Lighting and Power engine DG-12 and was caused by a cracked fuel nozzle tip. The fuel nozzle tip was manufactured from material lot number 150010 and initiat investigation indicates the problem was isolated. An investigation is still in process.

On March 20,1991, the C 8 Nuclear Engineering Department was notified by H.L.& P. that DG 13 had failed. Subsequent investigation has revealed that a cracked fuel nozzle tip caused the failure. The material lot from which the fuel nozzle tip was manufactured is 150006; Bendix was the manufacturer. Eddy current examination of other fuel nozzle tips from the same material lot identified similar problems.

This activity is in process.

A technical evaluation is being performed by Houston Lighting and Power. C B Reciprochting Nuclear Engineering will coordinate with Houston Lighting and Power and secure the necessary technical data for the final report.
