On May 27,1999, Units 1 and 2 were in Mode 1 at 100% power. On May 27,1999, Access Authorization personnel discovered that derogatory information on an individual's Personal History Statement had not been properly evaluated per South Texas Project's procedure " Unescorted Access Evaluation Process". The individual should not have been granted unesconed access in that information received May 24,1999 and reviewed on May 27,1999 met unescorted access denial criteria as specified in the procedure. Corrective actions include verifying the individual did not have current unesconed access at South Texas Project or at any other licensed utility, and l conducting Continuing Training for Access Authorization personnel on the requirements for evaluating l derogatory or potentially derogatory information.

DESCRIPTION OF EVENT:

On May 27,1999, Units 1 and 2 were in Mode 1 at 100% power.

On May 27,1999, Access Authorization personnel discovered that derogatory information on an individual's Personal History Statement had not been properly evaluated per South Texas Project's procedure "Unesconed Access Evaluation Process". Information received May 24,1999 and reviewed on May 27,1999 met unescorted access denial criteria for South Texas Project as specified in the procedure. A review of the individual's Nuclear File identified this information had been previously reported. The individual had been authorized unescorted access on three occasions (January 1997, August 1997, and March 1999) over the past 30 months In January 1997, contrary to the "Unesconed Access Evaluation Process" procedure, which required an

" Evaluation of Access Suitability / Unsuitability Form" due to derogatory information, the individual was granted Full unescorted access. During August 1997 the individual was processed again for unescorted access which included a review of the individual's file evaluation section. Based on the file not containing any evaluation of derogatory information, the individual was authorized unescorted access. On March I8,1999, the individual was processed again for unescorted access in support of Plant Outage IRE 08. This process included obtaining an updated criminal history check. The individual's fingerprints were obtained and submitted to the Nuclear Regulatory Commission on March 18,1999. The individual was authorized Temporary unescorted access on March 22,1999. On April 22,1999, the individual's access was revoked under favorable conditians. On May 24,1999, the individual's criminal history check results were received.

The review of the criminal history record determined the results could potentially meet unescorted access denial criteria and compensatory measures were initiated pending review of the individual's Nuclear File. On May 27,1999, a review of the file revealed that the results of the criminal history check received on May 24,1999 were the same as those received on March 27,1997. South Texas Project determined that information disclosed on the criminal history record should have resulted in access denial. A condition report l (99-8275) was generated and a 1-hour report was made to the Nuclear Regulatory Commission.

CAUSE OF THE EVENT:

The cause of this event was failure to follow procedure.

ANALYSIS OF EVENT:

Safeguards system failures include not only mechanical or electrical system failures but also improper security procedures or personnel practices. Discovered vulnerabilities include incidents in which the security system has not feiled, but some flaw in the security system that had existed without being noticed has been discovered. 1 l

ANALYSIS OF EVENT (continued):

Additionally, Nuclear Regulatory Commission Generic Letter 91-03 provides guidance for reporting safeguards events as required by 10CFR73, Appendix G. Ifit is determined that unescorted access would have been denied based on developed information, a 1-hour report is required after discovery of the new information as stated in REG GUIDE 5.62 and NUREG-1304.

Based on these reporting guidelines, this event was determined to be a 1-hour reportable event due to a l programmatic failure that resulted in an individual being authorized unescorted access on more than one l occasion with a criminal history record which met access denial criteria for South Texas Project. Accordingly, this event was reported to the NRC on May 27,1999.

This failure did not have an adverse effect on the health and safety of the public.

CORRECTIVE ACTIONS:

The following corrective actions have been or will be taken as a result of this event:

1. On May 24,1999, Access Authorization personnel verified the individual did not have current unescorted access at South Texas Project.
2. On May 24,1999, Access Authorization personnel verified the individual did not have current access at another utility through review of the Personnel Access Data System database.
3. Conduct Continuing Training for Access Authorization personnel on requirements for evaluating derogatory or potentially derogatory information. This will be completed by August 16,1999.

ADDITIONALINFORMATION:

There have been no other related events in the past three years that resulted in a reportable event.
