On May 21,1993 at approximately 1050, during testing of mode switch interlock functions, a question was raised regarding the mode switch position during this evolution. It was later detennined that in the past, the mode switch had been placed in the " Refuel" position during testing of switch interlock functions. Technical Specification Table 1.2 does not pennit this; therefore, this report is submitted pursuant to 10CFR50.73(a)(2)(i)(b) as a condition prohibited by the Technical Specifications.

The root cause for this event consisted of the initial interpretation of the Technical Specifications and the presence of conflicting information in stuveillance test procedures.

Corrective actions included revision of General Operating Procedure (GOP)-002 ' Plant Shutdown" to emphasize specific guidance concerning mode switch requirements, revision of other procedures to be consistent with the TS requirements, and tmining.

It was later detennined that in the past, the mode switch had been placed in the " Refuel" position during testing l of switch interlock functions. Technical Specincation Table 1.2 does not permit this; therefore, this report is submitted pursuant to 10CFR50.73(a)(2)(i)(b) as a condition prohibited by the Technical Specincations.

INVESTIGATION The investigation revealed that the practice of placing the mode switch in " Refuel" during Operational Condition 4 (Cold Shutdown) to perform nuclear instrumentation surveillances has been in place since initial plant startup. Interviews revealed that surveillance test procedures STPs provided that option. i During the performance of STP-503-4505 (SRM "C" weekly) on 5/21/93, Technical Specification Table 1.2 was reviewed and the mode switch was placed in the "Startup/ Hot Standby" position.

Strict compliance with the Technical Specifications was maintained. A preliminary review of past practice indicated that on March 11,1992 at 2156 the mode switch was taken to " Refuel" from the

" Shutdown" position to support SRM testing.

Interviews revealed that the option of placing the mode switch in " Refuel" existed in some of the instrumentation and control (I&C) STPs for nuclear instrumentation (STP-504-4501 to 4504).

(1) Revise General Operating Procedure (GOP)-002 " Plant Shutdown" to emphasize specific guidance from TS Table 1,2 concerning mode switch requirements during nuclear instmmentation testing. ,

(2) Brief all Senior Reactor Opemtors (SROs) and Reactor Operators (ROs) on the event and corrective actions, and emphasize that in the event of conflicting infonnation Technical Specifications shall always govern.

(3) Include this event in the next available training module for all SROs and ROs, with an estimated completion date of October 2,1993.

(4) Review all of the I&C nuclear instrumentation STPs and delete inappropriate refemnces to placing the mode switch in " Refuel" by November 1,1993.

SAFETY ASSESSMENT Placing the mode switch in " Refuel" during NI testing is a more conservative action than the Technical Specification allows, from a design standpoint. This is due to the fact that the design allows only one control rod to be withdmwn with the mode switch in " Refuel." Note that in "Stanup," the design permits more than one control rod to be withdmwn; however, two operators are used to verify rod position. Both methods are conservative; however, placing the mode switch in "Stanup/ Hot Standby" strictly complies with the Technical Specifications.
