The SS recognized this condition when he was documenting the successful completion of this surveillance in the daily surveillance tracking sheet. The frequency had been exceeded by a small fraction of the required 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> frequency. This STP verifies the operability of the airlock door seals 9s required by Surveillance Requirement 4.6.1.4.a.
The root cause of this event is inadequate Supervisory Methods which did not adequately tack and schedule the completion of STPs with a frequency of less than seven days.
Corrective actions include a review of associated Supervisory Methods and appropriate revisions to ADM-0015 to better define the division of responsibilities.
The satisfactory performance of the STP on April 14, 1994 demonstrated that the seals were operable )
and that the airlock doors were capable of performing their safety function during the 45 minutes that '
the required frequency was exceeded.

Prior to the discovery of this condition, the previous perfonnance of STP 057-3705 was documented on i l the daily surveillance tracking sheet by the SS at 2316 hours0.0268 days <br />0.643 hours <br />0.00383 weeks <br />8.81238e-4 months <br /> on Sunday, April 10,1994. This ;
surveillance is nonnally performed every Monday morning, Wednesday evening, and Friday evening.
l This routine has been in effect for the past year and has worked effectively. This schedule allows the l
! surveillance to be perfonned during slow traffic periods and spans the weekend to minimize the work I l
required during weekend periods. On Sunday night, April 10,1994, the Mechanical Supervisor on duty decided to perfonn the STP in lieu of performing it on Monday morning. This would allow Monday day shift to work on other items that were being perfonned in preparation for the upcoming refueling l outage. At this time no schedule adjustment was made to assure that the next STP perfonnance would ,
comply with the Technical Specification surveillance frequency. Consistent with routine practice, the ;
next STP was started Wednesday evening, April 13,1994 at 1917 hours0.0222 days <br />0.533 hours <br />0.00317 weeks <br />7.294185e-4 months <br />. The STP was performed I successfully and taken to the Control Room for approval. The SS documented the successful l completion of the surveillance on the tracking sheet at 0133 hours0.00154 days <br />0.0369 hours <br />2.199074e-4 weeks <br />5.06065e-5 months <br /> on April 14, 1994. A more detailed investigation of the actual surveillance performance times by reviewing key card ingress / egress times of maintenance personnel was also performed. This investigation determined that the actual performance of the surveillance l

ROOT CAUSE The root cause of this event is inadequate Supervisory Methods which did not adequately track and schedule the completion of STPs with a freyency of less than seven days. The methods used to schedule and track the perfonnance of this STP did not specify the required completion time for this surveillance. Therefore this process did not ensure a keen awareness of the required completion time.
I One similar LER has previously been submitted by RBS that involved a late surveillance for an activity that was required to be perfonned on a frequency of less than seven days. This event was reported as LER 86-048. The causal factors associated with LER 86-048 were a poor scheduling process and insufficient accountability for ensuring that daily surveillances were performed on schedule. The particular surveillance that was missed in LER 86-048 was STP-500-4550, "RPCS High and Low Power Setpoint Functional Test." The corrective action was the creation of a daily schedule / checkoff sheet of daily STPs that required the Control Operating Foreman (COF) and Shift Supervisor signoffs.
The corrective actions associated with LER 86-048 have been in place and have been effective in assuring the completion of short frequency surveillances on schedule. However, the division of responsibility was not fully developed and the scheduling practices focused primarily on sarveillance I frequencies without adequate emphasis on the associated " drop dead" times.
CORRECTIVE ACTION
- 1. Individual departments will be responsible for scheduling, tracking and implementation of STPs assigned to theii functional area. Operations will maintain an oversight role to assure that STPs are performed in a timely fashion. ADM-0015 will be revised to more i clearly define these responsibilities.
- 2. Each responsible department will review the supervisory / tracking practices for weakness for their functional area (Operations, Security, Chemistry, Reactor Engineering, and Maintenance) which are related to STP's with frequencies less than seven days and event-related STP's. In-house tracking processes will be created / revised as necessary to preclude missed STP's.
- 3. Mechanical Maintenance has developed a daily tracking process for STP-057-3705 with last perfonned date and time and the next due date and time.

- 4. Personnel in the responsible departments will be briefed on the revisions to ADM-0015 as described in item 1 above. Additionally, the Maintenance Department personnel will be briefed on this event, the corrective actions, and the importance of complying with scSeduled dates and times for surveillances.
l SAFETY ASSESSMENT l
The purpose of this surveillance is to ensure that leakage from primary containment via the airlocks is well within the leakage limits assumed in the design basis of the plant. The satisfactory performance of the STP on April 14,1994 demonstrated that the seals were operable and that the airlock doors were capable of perfonning their safety function during the 45 minutes that the required frequency was exceeded.
