had not been performed after replacement of an inflatable seal was completed on 2/5/89 per Maintenance Work Order Request (MWOR) R133511. -

l This event is reportable per 10CFR50.73 (a) (2) (1) (B) as an operation prohibited by Technical Specification 3/4.6.1.3.

On reviewing the MWOR it was determined that Surveillance Test Procedure (STP)-057-3804 had been inadvertently deleted and replaced with STP-057-5203. The root cause was determined to be personnel ,

error by the maintenance personnel. The corrective maintenance  ;

procedure (CMP)-9249, " Personnel Air Locks Door Seal Requirement", was revised such that required testing is specified in the acceptance l criteria determination section of the procedure.

l L During the period 2/5/89 and 3/15/09, no event challenging containment l integrity occurred. STP-057-3704 was satisfactorily performed on 3/15/89 as a normal surveillance at the start of the second refueling

, outage. The overall leak rate would have been found satisfactory on

! 2/5/89 had the STP been performed on that date. The health and safety .

of the public was not significan*.ly affected by this event.

O 13 19 0 10 Ol2 0F 013 vins,, . . == aacwamamm REPORTED CONDITION At 0800 on 11/4/89 with the unit at 95 percent power in Operational Condition 1, it was discovered that a required post-maintenance overall leak rate test had not been performed on the upper containment airlock (*AL*) after replacement of an inflatable seal was completed on 2/5/89 per Maintenance Work Order (MNO) R133511. The door was placed in service at 1718 on 2/5/89 without proving operability in accordance with the overall leak rate Surveillance Test Procedure (STP) -0 5 7-3 7 0 4 . This event is being reported as an operation prohibited by the Technical Specifications pursuant 10CFR50. 73 (a) (2) (1) (B) .

INVESTIGATION The Maintenance Work Order Request (MWOR) for installation.of the new seal was reviewed. It was determined that STP-057-3704 was originally listed on the MWOR as one of the required operability tests following replacement of the seal. This surveillance was inadvertent 1/ deleted and replaced on the MWOR by the airlock seal accumulator surveillance

- test (STF-057-5203). Maintenance personnel should not have changed the STPs. The root cause for not performing STP-057-3704 is personnel error.

CORRECTIVE ACTION The corrective maintenance procedure (CMP)-9249, " Personnel Air Locks Door Seal Replacement", was revised auch that required testing is specified in the acceptance criteria determination section for the procedure. This procedure change included reference to all three tests, " Drop Test", " Airlock Seals Accumulator Test", and " Personnel Air Locks Door Seal Replacement Test", and as such assures that all required testing will be performed in the future.

In reviewing earlier LERs involving missed STPs performed for operability purposes and that were caused by personnel error, no earlier LERs were identified which involved MWOR document changes; nor were any identified which involved airlock door seal replacement.

Corrective action taken on other missed STP LERs involving personnel error would not have prevented occurrence of this event.

SAFETY ASSESSMENT STP-057-3704 was satisfactorily performed on 3/15/89 as a normal surveillance at the start of the second refueling outage. Since no i additional maintenance was performed on the airlock between 2/5/89 and 3/15/89, the overall leak rate would have been found to be satisfactory on 2/5/89, had Lne STP been performed on that date.

E During this period, no actual event occurred which would have l challenged the system. As such, the health and safety of the public l was not significantly affected by this event.
