Radiation Monitoring Instrumentation - Containment Purge and Pressure / Vacuum Relief Isolation System - Inoperable.

This report was initiated by Incident Report 82-503.

CONDITIONS PRIOR TO OCCURRENCE:

Mode C - Rx Power 0% ~ - ~ Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

At 0411 hours0.00476 days <br />0.114 hours <br />6.795635e-4 weeks <br />1.563855e-4 months <br />, December 6, 1982, during routine refueling operation, the Control Room Operator discovered that the Containment Air Particulate Monitor 1RllA indication had failed low. The channel was declared inoperable; since it is utilized by the Containment Purge and Pressure / Vacuum Relief Isolation System, Limiting Condition for Operation 3.3.3.lb Action 22 was entered. The containment air lock doors were immediately shut to further isolate the containment purge flowpath. The related containment gaseous and iodine monitors (Channels 1R12A and 1R12B) were operable providing redundant indication and purge isolation capability.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Investigation of the problem revealed that the channel scintillation detector had failed. No other problems were evident, and no recent, similar occurrences had been noted. The event was therefore assumed to be of an isolated nature.

ANALYSIS OF OCCURRENCE:

The operability of the Containment Purge and Pressure / Vacuum Relief .

Isolation System insures that the containment vent and purge penetrations will be automatically isolated upon detection of high radiation levels within the containment. This feature is required to1 restrict the release of radioactive material from the containment atmosphere to the environment in the event of an accident.

. The. failed' detector was replaced and the channel.was satisfactorily tested. Radiation Monitor 1RllA was declared operable at 2040 ,
