Investigation revealed that the Air Particulate Detector (APD) l loIs1 I sample line to the plant vent had been erroneously capped during partial completion ofl I O l6 l l Design Change Request (DCR) ED-0014.

Subsequent investigation showed the vacuum relief l o l 7 l l device on the suction of the APD was lifting at normal operating pressure in the l

i o ;a ; ; suction line, and the monitor had been inoperable since prior to the unit start-up in l o April of g yyear.
