Page 1 of I Docket No. 50-338 Report No. LER 81-014/03L-0 Description of Event On March 25, 1981, during Type "C" testing, it was noticed that the closure weight on check valve 1-RS-18 was net installed. This would prevent positive seating of the check valve when differential pressure in the direction of flow is < 1.2 psid.

Probable Consequences of Occurrence Failure of the check valve to fully seat during normal operation conditions would provide a potential leakage path from the containment.

However, the outside recirculating spray subsystem is a closed system since the suction and discharge piping terminate inside of the containment. Therefore, the health and safety of the public were not affected.

Cause of Event The cause of this event is unknown. The time of removal of the weight could not be determined. During the refueling outage there was maintenance conducted in the area and, possibly, the weight was removed to clear an

(]) obstruction. However, this could not be verified by documentation nor by interviews with maintenance personnel.

Immediate Corrective Action A valve weight was manufactured and installed. The valve was sub-sequently tested as per the Type "C" testing program with satisfactory results.

No further action required.

Actions Taken to Prevent Recurrence The need for formally documenting the removal of obstructions by initiating a work request will be re-emphasized to each maintenance department.
