s TOLEDO EDISON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-77-19 l

l DATE OF EVENT: July 8, 1977 FACILITY: Davis-Besse Unit 1.

IDENTIFICATION OF OCCURRENCE: The remote hydraulic fluid reservoir for two l

l hydraulic snubbers, I.D. No. GCB-7-H4, was found empty.

Conditions Prior to Occurrence: The plant was in Mode 4.

Power:

(MWE) = 0 and (MWT) = 0.

Descripcion of Occurrence: At approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on July 8, 1977, station personnel labeling and inspecting hydraulic snubbers found an empty remote hydrau-lic fluid reservoir for the two (2) hydraulic snubbers (I.D. No. GC3-7-H4) on the Decay Heat System normal cooldown line. These snubbers dampen dynamic pipe movements which could. occur in a seismic occurrence to protect the piping. This placed the station in the Action Statement of Technical Specification 3.7.7.1.

Desianation of'Accar nt Cause of Occurrence: ' Maintenance personnel found no hydrau-lic fluid contained in the remote reservoir or in the tubing going to the snubber.

There was no indication of fluid on the floor.

Due to the lack of evidence of

. hydraulic fluid in the reservoir or any leakage from the snubbers and fittings,-it is apparent that the remote reservoir was never filled by Construction personnel i

who were responsible for initial installation -of the snubber.

Analysis of Occurrence: These snubbers were placed on the decay heat line to l

prevent excessive movement during a seismic event.

Since no seismic events have occurred, there was no danger to the health and safety of the public or i

station personnel.

Corrective Action: Maintenance personnel filled the remote reservoir, purged the two hydraulic snubbers and declared the snubbers operable by 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> on i '

July'9.

This removed the station from the Action Statement of Technical Speci-fication 3 7.7.1.

No similar occurrence is' expected since all hydraulic snub-bers have aow been inspected for adequate hydraulic fluid level.

Failure Data:- No previous similar events have occurred.
