m TOLEDO EDI' SON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR PC'n'ER STATION SUPPLE 2 ENTAL INFORMATION FOR LER NP-33-77-69 DATE OF EVENT: August 23, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Hydraulic Snubber E3D-19-H144 inoperable.

Conditions Prior to Occurrence: The plant was in Mode 3, with Power (MWT) = 0 and Load (MWI) = 0. .

Description of Occurrence: At 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br /> on August 23, 1977, Hydraulic Snubber E3D-19-H144 was discovered upside down, rendering the hydraulic snubber inoperable.

The hydraulic snubber is located on Auxiliary Feedw: ster Loop 2. This placad the Station in the Action Statement of Technical Specification 3.7.7.1 which requires all nuclear safety related hydraulic snubbers to be operable in Mode 3.

Designation of Apoarent Cause of Occurrence: A loose lock nut (which was possibly due to 1= proper initial installation during plant construction) on the hydraulic snubber extension piece allowed the hydraulic snubber ec rotate placing the hydraulic fluid reservoir on the bottom. The hydraulic fluid reservoir =ust be above the hydraulf: snubber cylinde.r in order for the hydraulic snubber to be operable.

s Analysis of Occurrence: There was no threat to the health and safety of the public or Station personnel. Auxiliary Feedwater Loop 1 was available for service.

Corrective Action: The hydraulic snubber..was M4 =ennnected, purged to eliminate any air in the hydraulic fluid and tested. Afrer the hydraulic snubber was re-installed, the lock nut on the extension piecesuas tightened. The hydraulic snubber was declared operable at 2320 hours0.0269 days <br />0.644 hours <br />0.00384 weeks <br />8.8276e-4 months <br /> on August 23, 1977, removing the Station from the Action Statement of Technical Specification 3.7.7.1.

Failure Data: There has been no previous failure with Hydraulic Snubber E3D-19-H144 There was a previous failure with Hydraulic Snubber GC3-7-H4 which was found with no hydraulic fluid in the remote reservoir as reported on Report No. NP-33-77-19.
