SUPPLEMENTAL INFORMATION FOR LER NP-33-77-62 DATE OF EVENT: August 25, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURPINCE: Auxiliary Feed Pump 1-1 deliberately rendered inoperable to replace the relief valve in the cooling water line.
Conditions Prior to Occurrence: The plant was in Mode 3 with Power (MWT) = 0, and Load (MWE) = 0.
Description of Occurrence: At 0935 hours0.0108 days <br />0.26 hours <br />0.00155 weeks <br />3.557675e-4 months <br /> on August 25, 1977, Auxiliary Feed Pump 1-1 cooling water was deliberately isolated which rendered the pump in-operable. This was done to install the pernanent pressure relief valve PSV 4979 on the cooling water discharge line from the pump bearings as per Construe:1on Work Permit CWP-50-M-74.
At 0945 hours0.0109 days <br />0.263 hours <br />0.00156 weeks <br />3.595725e-4 months <br />, August 30, 1977, Auxiliary Feed Pu=p 1-2 was rendered inoperable for the same purpose. This placed the unit into the Action Statement of Technical Specification 3.7.1.2.
This Technical Specification re-quires that both Auxiliary Feed Pumps be operable in Modes 1, 2 and 3.
O Designation of Apparent Cause of Occurrence: This relief valve was installed to replace the original relief valve since the original lacked proper documentation.
The new relief valve was not delivered on site until earlier this month.
Analysis of Occurrence: There was no threat to the health and safety of the public or to Station Personnel. One Auxiliary Feed Pump was operable at all times.
I Corrective Action: At 1615 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.145075e-4 months <br /> on August 26, 1977, the relief valve installa-tion was completed, cooling water was restored to the Aax111ary Feed Pump 1-1, and the pump was declared operable. At 1625 hours0.0188 days <br />0.451 hours <br />0.00269 weeks <br />6.183125e-4 months <br /> on August 30, 1977, Auxiliary' Feed Pump 1-2 was returned to service. This removed the unit from the Action Statement of Technical Specification 3.7.1.2.
Failure Data: No previous events have occurred due to this cooling water relief valve.
