Voluntary Report of a Missed Daily Test of the Portable Fire Detection System On July 28, 1989, Toledo Edison submitted a letter to the NRC (Serial No. 1689) describing a portable fire detection system intended to reduce certain continuous fire watches to hourly fire vatch patrols. The letter included the planned compensatory actions for the deviations from the NFPA 72D code. The actions included a test at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the auto-dialer, message unit, and phone lines.

Contrary to the above, on February 4, 1990, the daily functional test was not done. The hourly fire vatch check of the local panel for alarms and to assure the phone line was connected was still being performed. On February 5, 1990, it was recognized that the daily functional test was missed. The functional test was performed, which verified that the circuit between the local panel and the control room was still functioning.

The test was missed due to a personnel error on the part of the Shift Supervisor who inadvertently failed to ensure the test had been done. The test activity vas on the Shitt Supervisor's turnover sheet. The test had usually been performed by the Shift Manager, but upon entry into Mode 5 and the commencement of the Sixth Refueling Outage, the Shift Manager was no longer a required member of the shift compliment. The veekend that the test was missed, no Shitt Manager vas scheduled to work.

To ensure this daily functional test of the portable detection system is performed, the requirement vill be added to the Oparations Department Monthly Activity Log, DB-0P-00003 This document identifies tasks usually of a repetitive nature and includes regulatory requirements, commitments, andItgood operating practices to be accomplished by Shilt Operations personnel.

identifies the frequency for the activities and provides space for a checkoff when the activity is complete. This activity was added to the log as of March 9, 1990.
