Virginia Electric and Power Company North Anna Power Stat su, Unit #1 At tachme n t , Page 1 of 1 Docke t No. 50-338 Report No. LER 80-004/03L-0 Description of Event During required testing to verify containment integrity, the outer door of the equipment hatch escape lock was opened by mistake. When the door was opened, the lock was found to be under a slight vacuum which gave indication that the inner door seal was leaking. Subsequent testing of the escape lock door seals revealed that the inner door seal was, in fact, leaking and that the outer door seal was leaking as well.

Prcbable Consequences of Occurrence The consequences of this event were limited because when the leakage was discovered, the unit was in Mode 4 and prior to that time had been shutdown for refueling. The effects were also minimized because the integrity of the containment was restored within the time limit allowed by the Action Statement of T.S. 3. 6.1.1. As a result, the health and safety of the general public were not affected by this event. There are no generic implications associated with this occurrence.

Cause of Event It is believed that the inner door seal was previously disturbed when someone inadvertently attempted to open the door with the strongback on. The leakage of the outer door seal was attributed to traffic through the door when working on che inner coor. Investigation of this event revealed that *he door seals were not testei af ter entry into the emergency hatch to remove the strongback from the inner door.

Immediate Corrective Action The gasket for the inner door was removed , cleaned and reinstalled. The outer door gasket was replaced with a new set. Both doors were then properly adjusted and the seals were retested with acceptable results.

Scheduled Corrective Action No scheduled corrective action is required.

Actions Taken to Prevent Recurrence A program will be developed to ensure the correct sequence of future testing o' the escape lock.
