On December 13, 1978, while the periodic test for the Post DBA Hydrogen Analyzer (H2 -HC200) was being performed, it was discovered that the analyzer indicated 8.5% less hydrogen than the actual sample gas indicated. Due to the 8.5% difference between the analyzer and the sample, 2-HC-HC-1 was declared inoperable. Technical Specification Statement 3.6.4.2 requires that two separate containment hydrogen recombiners shall be operable.

Probable Consequences of Occurrence:

As a result of 1-HC-HC-1 being operable, no degradation of the containment hydrogen removal systems functions were expected. The public health and safety were not endangered by the inoperability of 2-HC-HC-1 since 2-HC-HC-1 was returned to service within 30 days.

Causes of Occurrence The cause of the occurrence was due to the analyzer H 2A-HC-200 being out of calibration.

Immediate Corrective Action:

To ensure that no flow entered the recombiner 2-HC-HC-1, the flow from the Unit 1 containment wa; isolated from 2-HC-HC-1. Once the flow had been isolated, a maintenance report was submitted to the instrument shop. The work performed under the maintenance report was to recalibrate and retest the analyzer. During the retest, the analyzer and the sample gas indicated the same percentage, recombiner ?-HC-HC-1 was placed back into operable status.

No further action required.

No further action required.
