• PSI Overview Program

Dear Mr. Hawkins:

In accordance with the preliminary comf t=ents made by PSI in the June 26, 1979 meeting (Ref: Attached Pre-Meeting Attendance List),

PSI will establish a 100I " Overview Program" encompassing safety related concrete activities performed by the Gust K. Newberg Company. The program v111 consist of the following:

1. Cust K. Newberg has upgraded certain work and quality control procedures; based on the revisions, PSI has instituted a " Hold Point" program consisting of activities that cannot be gone beyond without PSI QC verification that the activity has been accomplished correctly. This verification will consist of actual physical inspections and surveillances performed and documented in accordance with approved procedures.

2. The " Hold Polui.<" will be transmitted to the Newberg QA Supervisor on a psriodic oasis or until such time the " Hold Points" are changed or racinded.

3. Each " Hold Point" is predefined (See Attac1 ment B).

4. The " Hold Point" program uns instituted July 3,1979 in order to

" burn in" the system to demonstrate it's effectiveness.

5. PSI has augmented it's QC inspection and engineering staff (2 QC Engineers andX1evel IIycrete Inspectors) to perfon or surveil theactivities} t t . O. c ! Z

6. PSI has performed and documented an indoctrination review and training session of the " Bold Point" program as well as the revised-Newberg work and Quality Control procedures.

Dear Mr. McLarty:

As a result of the meetings on June 27, 1979, between N-MH and PSI res9ective Q. A. Department heads and a subsequent meeting between N-MH Q. A. and N-MH project management, we have deter-mined that the following hold peint criteria will be placed into offect as directed in writing by PSI:

5. Hold Point No d As stripping is in progress, the N-MH Q. C. Inspector will in-spect the area and complete Part I of the Honeycomb Report in accordance with QAPN-10. Copy of this preliminary report will be transmitted to PSI. When the area is ready for pre-paration pleted. NOTE:

inspection, PSI will be notified and Part II com-The amount of stripping in any area will be controlled by subsequent operations and may only be partial, in which case a new Honeyccmb Report if required will be initiated when the balance of forms are ramnoved.

Sign off of all hold points by the PSI Representative will be indicated on the form that applies to the applicable hold point. .
