Oil leak in the South Penetration area.

CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 2, Reactor Power 3.5%, Plant Pressure 2235, Plant Temperature 548°F.

DESCRIPTION OF OCCURRENCE:

On 12/29/76, hydraulic oil leaked from No. 12 Main Steam Isolation Valve (12MS167) which rendered the isolation valve inoperable in the closed position. The Shift Supervisor implemented Technical Specifications Action Statement 3.7.1.5. The Fire Brigade was dispatched to the south penetration .area. The insu_lation around 12MS167 was rerµoved and the oil cleaned up. The plant was taken from Operational Mode 2 to Mode 3 to cool the main steam header down.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The hydraulic oil line on 12MS167, which runs from the operating solenoid to the spool block on the high pressure side of the valve operator, fractured and caused the reservoir to drain in the area of the No. 12 Main Steam Stop Valve.

ANALYSIS OF OCCURRENCE:

With one main steam line isolation valve inoperable, .subsequent operation in Mode 1, 2, or 3 may proceed provided the isolation valve is maintained closed. At no time was there any hazard to the general public or sl.te personnel.

CORRECTIVE ACTION:

The hydraulic line on 12MS167 was replaced with one from Unit No. 2.

Investigation is being made to obtain piping with a greater wall thickness.

FAILURE DATA:

This is the first occurrence of this type.
