Box 236, Hancocks Bridge, New Jersey 08038-0236 DEC 1 3 2004 LRN-04-0579 -PSEG NuclearLLC United States Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 Special Report 311104-010 Salem Unit 2 Facility Operating License DPR-75 Docket No. 50-311 This special report is being submitted pursuant to the requirements of Salem Unit 2 Technical Specification (TS) 3.3.3.1 "Radiation Monitoring Instrumentation" Action b, due to the Noble Gas Effluent monitors (2R45) being inoperable for greater than 7 days.

On December 1, 2004 at 20:45, TS 3.3.3.1 was entered due to the 2R45 detector being inoperable. TS 3.3.3.1, Action b requires that " With one or more radiation monitoring channels inoperable, take the ACTION shown in Table 3.3-6." Action 26 of Table 3.3-6 requires that within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> the preplanned alternate method of monitoring the appropriate parameter(s) be initiated, and (a) the inoperable channel be restored to operable within 7 days, or (b) to prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status.

The preplanned alternate method of sampling was implemented within the 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> required by technical Specifications; however the detector could not be returned to Operable within the required seven days.

PSEG could not restore the detector to operable because of:

1. Less than adequate administrative controls The original notification was inadvertently left out of the schedule; therefore technicians were not assigned in a timely fashion.
2. Parts availability Troubleshooting determined that the skid sample pump needed to be replaced.

A spare skid sample pump is not available.

Document Control Desk LRN-04-0579 PSEG expects to return the detector to an operable status as soon as the replacement part is acquired. In the meantime the Technical Specifications compensatory actions will remain in effect.
