No. 11 Steam Generator Feedwater Flow Channel II Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1, Reactor Power 99%

DESCRIPTION OF OCCURRENCE:

At 2040 hours0.0236 days <br />0.567 hours <br />0.00337 weeks <br />7.7622e-4 months <br />, during functional testing, No. 11 Steam Generator Feed-water Flow Channel II was declared inoperable due to the trip point being above the.Technical Specification limit. Action Statement No. 7 for LCO 3.3.1.1 was implemented and at 2335 hours0.027 days <br />0.649 hours <br />0.00386 weeks <br />8.884675e-4 months <br />, instrumentation per-sonnel returned the channel to operable status.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

This occurrence was the result of a failed operational amplifier in the dual comparator which resulted in the trip setpoint being above

  • the Technical Specification limit.

ANALYSTS OF OCCURRENCE:

~echnical Specification 3.3.1.1, Action Statement No. 7, states that "with the number of operable channels one less .than the total number of channels, startup and/or power operation may proceed until perform-ance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />".

No. 11 Steain Generator Feedwater Flow Channel II was placed in the tripped con_dition with-in 1 -hour* and remained in that condition until

'it ~as returned to operable status.

  • CORRECTIVE ACTION:

.Four operational amplifiers were replaced in the Hagan Dual Output Comparator. The comparator was then replaced. The functional test

-_was then completed to confirm operability.
