Missed Surveillance: Non-Radwaste Sampling for Suspended Solids Analysis CONDITIONS PRIOR TO OCCURRENCE:

N/A DESCRIPTION OF OCCURRENCE:

On the occasions summarized below, treated effluent was released from the non-radioactive liquid waste treatment basin, but a sample was not collected for suspended solids analysis.

January 1, 2, 7, 10, 14, 15, 16, 21, 24, 25, 26, 27, 28, 29, 30 February 7, 21 (Ref.: Section 2.2.2 of Appendix "B" to Operating License DPR-70)

The missing samples were identified upon records review in light of Occurrence 78-10/04L.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was personnel error. Corrective action as stated in LER 78-10/04L had not yet been implemented.

ANALYSIS OF OCCURRENCE:

N/A CORRECTIVE ACTION:

Responsibility to collect samples was re-assigned from demineralizer plant operator (day shift) to midnight shift Health Physics Technician.
