Inoperable Containment Personnel Airlock CONDITIONS PRIOR TO OCCURRENCE:

Hot Standby Mode 3 DESCRIPTION OF OCCURRENCE:

During containment entry in Mode 3, a Maintenance Supervisor reported a failure of the interlock assembly on the 100 foot elevation person-nel airlock. Action Statement for LCO 3.6.1.3 was implemented.

Maintenance personnel reworked the door and retest results were satisfactory.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was failure of two (2) cam followers

.on the interlock device. Cam follower failure resulted in neither door opening.

ANALYSIS OF OCCURRENCE:

Technical Specification 3~6.1.l states that with an airlock inoper-able, restore the airlock to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or be in at least Hot Standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />. The elevation 100 foot interior door was reworked and the airlock returned to operable status in 33 hours3.819444e-4 days <br />0.00917 hours <br />5.456349e-5 weeks <br />1.25565e-5 months <br />.

The unit remained in Mode 3 throughout this period.

CORRECTIVE ACTION:

The failed cam followers were replaced from spares and the interlock was tested satisfactory. Door seal leak rate was performed to con-firm operability. A design change request has been initiated to upgrade the door mechanism.
