Control Bank and Shutdown Rod Misalignment Control Banks: A, B, C, D Shutdown Rods: lSAl 1SA3 CONDITIONS PRIOR TO OCCURRENCE:

Startup Mode 2; Reactor Power: 1%

DESCRIPTION OF OCCURRENCE:

At 0050 hours5.787037e-4 days <br />0.0139 hours <br />8.267196e-5 weeks <br />1.9025e-5 months <br />, control room personnel identified control banks A, B, C and D and shutdown rods lSAl and 1SA3 as being greater than 12 steps below the bank demand position. Technical Specification 3.1.3.l(b) was implemented. At 0649 hours0.00751 days <br />0.18 hours <br />0.00107 weeks <br />2.469445e-4 months <br />, the reactor was tripped from 1% power to comply with the Action Statement. At 1145 hours0.0133 days <br />0.318 hours <br />0.00189 weeks <br />4.356725e-4 months <br />, the above control and shutdown rod position indicators had been recalibrated and Action Statement 3.1.3.1 was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Control bank position indicators had not been calibrated at the operating position. When control banks A, B, C and D were with-drawn to the critical position, the indicators were greater than 12 steps from the bank demand.

Shutdown rods lSAl and 1SA3 position indicators had drifted to greater than 12 steps.

Non-linear scales and instrument drift was the cause of this occurrence.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.1.3.1.b states that with more than one full or part length rod inoperable or misaligned from the bank demand posi-tion by more than + 12 steps (indicated position), be in Hot Standby within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.
