The cause of this occurrence was personnel error. The responsible technician* failed to take the required suspended solids samples.

To* prevent reoccurrence of this type of event, a daily check list is being developed that will require all samples to be signed off and reviewed by a supervisor. This system is expected to be fully imple~
mented by August 15, 1978.
