4/4/78 occurrence Date: 2/17/78 Facility: Salem Generating Station Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

During 1977, contrary to Environmental Technical Specification Section 3.1.1.4, monthly water quality samples were taken at the intake and at a point outside and downstream of the mixing zone, but were *not analyzed for BOD 5 , Kjeldahl nitrogen, phenols, sodium and potassium.

In June and July 1977, monthly water quality samples were taken at the discharge but were not analyzed for BODs or Kjeldahl nitrogen.

The missing data was identified in a routine annual program review on February 17, 1978 prior to submitting the 1977 Annual Non-radiological Environmental Operating Report.

CONDITIONS PRIOR TO OCCURRENCE:

N/A DESCRIPTION OF OCCURRENCE:

1. Monthly samples were taken at all three stations as required by Environmental Technical Specifications, but only the samples collected at the discharge were analyzed for BOD 5 , Kjeldahl nitrogen, sodium, potassium, and phenols.
2. In June and July 1977, the BOD 5 and Kjeldahl nitrogen samples from the discharge were not analyzed by a consulting laboratory since recommended holding times had been exceeded before the consulting laboratory received the samples.
  • DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

1. Personnel responsible for sample analysis were not aware of written instructions approved on April 15, 1977.
2. Consulting laboratory personnel were not alerted that a sample had been deposited in their after-hours holding box.

CORRECTIVE ACTION:

1. Analysis personnel were reminded of the written procedure verbally on February 17, 1977. Written followup correspondence is in preparation.
2. PSE&G personnel subsequently telephoned consulting laboratory at the beginning of the next working day after sample delivery.
