Pressurizer Level Channel I Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 3, Tave 547°F, Pressure 2235 psig DESCRIPTION OF OCCURRENCE:

At 0700 hours0.0081 days <br />0.194 hours <br />0.00116 weeks <br />2.6635e-4 months <br />, pressurizer level channel I was declared inoperable due to it reading 3 to 5% higher than other channels. Pressurizer level channel I was placed in the 'tripped condition. Action State-ment No. 7 for LCO 3.3.1.1, Table 3.3.1 and Action Statement A for LCO 3.3.2.l were placed in effect. Power operation continued until 5/18/77 when the unit was taken out of service to repair the channel.

DESIGNATION OF APPARENT* CAUSE OF OCCURRENCE:

Two problems were identified that caused this occurrence. Gas was trapped in condensate pot causing a false high level signal. Addi-tionally, the low pressure isolation valve and the equalizing valve were found to be leaking through. With the equalizing valve leaking through a false differential pressure signal was being applied to the transmitter.

ANALYSIS OF OCCURRENCE:

Action Statement No. 7 for LCO 3.3.1.1 and Action Statement A for LCO 3.3.2.1 allow operation to continue until the next required functional test provided the inoperable channel is placed in the tripped condi-tion within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />. Channel I pressurizer level was placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, therefore, the LCO was not violated.

The function test for this channel was due on 5/19/77. At no time was there any danger to the health and safety of the general public or site personnel.

CORRECTIVE ACTION:

On 5/18/77, the unit was placed in Mode 2 to facilitate venting of the pressurizer level condensate pot. The pot was vented and the indica-tion decreased to approximately 3%. The equalizing valve and low side

  • e isolation valve were replaced and the channel output came to within 2 millivolts of other operable channels. Channel I was declared operable and all action statements terminated.
