On August 5, 1996, with the plant operating in mode 1 at 100% power, an internal Nuclear Assessment Section (NAS) assessment identified that test documentation for the October 5, 1995 performance of OST-1052 (Reactor Auxiliary Building (RAB)

Emergency Exhaust System, 18-month Operations Surveillance Test) had not been transferred to the nuclear records storage vault and could not be located within the plant's document control program. Records and completed test documentation showed that OST-1052 had last been performed on April 24, 1994. The plant's surveillance test scheduling system (STSS) indicated successful performance of OST-1052 on October 5, 1995 during refueling outage six. However, NAS personnel were unable to locate the October 5, 1995 OST-1052 test documentation. During the assessment, Engineering personnel informed NAS that they were also unable to locate the October 5, 1995 OST-1052 test documentation during RAB Emergency Exhaust System testing performed in May of 1996, but did not consider the possibility that the test had not been performed, which would result in a Technical Specification violation. Additional searches were performed by personnel responsible for scheduling and tracking surveillance tests, and'personnel involved in processing completed surveillance test documentation. These searches also failed to locate the October 5, 1995 OST-1052 test records.

Subsequent investigation could not verify that OST-1052 was performed in October 1995 as indicated by STSS, thus resulting in a violation of the Technical Specification surv'eillance interval on March 11, 1996, which is reportable per 10CFR50.73. This violation was caused by personnel error during the surveillance test documentation close-out process. OST-1052 was successfully performed on June 6, 1996, which verified the operability of the Reactor Auxiliary Building E-6A Emergency Exhaust System Fan. Corrective actions included performing an audit and reviewing other surveillance test documentation to ensure no other discrepancies existed. Additionally, this event was included in the STSS training lesson plans for Operations and Work Control personnel.

EVENT DESCRIPTION:

On August 5, 1996, with the plant operating in mode 1 at 100% power, an internal Nuclear Assessment Section (NAS)

Assessment (H-OI 96-01) identified that test documentation for the October 5, 1995 performance of OST-1052 (Reactor Auxiliary Building Emergency Exhaust System, 18-month Operations Surveillance Test) had not been transferred to the nuclear records storage vault and could not be located within the plant's document control program. Records and completed test documentation showed that OST-1052 had last been successfully performed on April 24, 1994. However, the plant's surveillance test scheduling system (STSS) indicated a successful performance of OST-1052 on October 5, 1995 during refueling outage six. NAS assessment personnel were unable to locate the October 5, 1995 OST-1052 test documentation during the assessment.

During the assessment, Engineering personnel informed NAS that they were unable to locate the October 5, 1995 OST-1052 test documentation for use as base-line information during RAB Emergency Exhaust System testing performed in May of 1996. The involved Engineering personnel but did not consider the possibility that the test had not been performed, which would result in a Technical Specification violation.

To resolve this condition, additional searches were performed by personnel responsible for scheduling and tracking surveillance tests, and personnel involved in processing completed surveillance test documentation. These searches also failed to locate the October 5, 1995 OST-1052 test records.

Subsequent investigation could not verify that OST-1052 was performed on October 5, 1995 as indicated by STSS, thus resulting in a violation of the Technical Specification surveillance interval on March 11, 1996, which is reportable per 10CFR50.73. OST-1052 was successfully performed on June 6, 1996, which verified the operability of the Reactor Auxiliary Building E-6A Emergency Exhaust System Fan.

CAUSE:

The cause of this event was personnel error during the surveillance test documentation close-out process for OST-1052.

Investigation could not conclusively determine why STSS indicated a successful performance of OST-1052 on October 5, 1995, but the most probable explanation is that the STSS task number associated with OST-1052 was mistakingly included by Operations personnel on the list of tests performed that day. Specifically, during the October 5, 1995 time frame, the plant was completing Refueling Outage 6 and a large number of STSS tasks (surveillance tests) were being performed each day and listed on a generic STSS task sheet list. The STSS coordinator then entered test completion dates into the program and established the next testing due date. During this process, it is believed that OST-1052 was included on the October 5, 1995 generic STSS completed test list, but was not actually performed.

SAFETY SIGNIFICANCE:

There were no adverse safety consequences as a result of this event. Even though OST-1052 was overdue on March 11, 1996 and not successfully performed until June 6, 1996, other surveillance testing was successfully performed on a monthly basis which provides confidence that the RAB Emergency Exhaust System (EIIS Code: VF) would have performed as needed in the event of a design basis accident. OST-1032 and OST-1049 are the "A" and "B" train (respectively) monthly RAB Emergency Exhaust System operability verification tests and were performed in March, April and May 1996. These tests initiated an RAB Emergency Exhaust System start from the control room and ran the E-6 A & B fans for 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> with the associated heaters energized. Although differential pressure between the RAB and outside atmosphere is not measured by the monthly tests, the system was verified to be functional by running the exhaust fans.

PREVIOUS SIMILAR EVENTS:

There have been no previous missed surveillance tests due to the completion of an incorrect STSS task sheet during the close out process for surveillance test documentation.

CORRECTIVE ACTIONS:

l. A sample of completed surveillance test documents and associated STSS records for the period of January 1995 through June 1996 were reviewed. This was completed on September 4, 1996 and determined that that no other similar discrepancies existed.
2. A new surveillance testing scheduling process has been developed and implemented on a trial basis. The lesson plans used for training Operations and Work Control personnel on this new scheduling process includes a review of the event contained in this LER to emphasize the importance of accurate information entry and coordination.
