On March 12, 1996, surveillance testing was performed on the Containment Building Personnel Air Lock (PAL)

Barrel to satisfy the six month type-8 local leak rate testing required by Technical Specification 4.6.1.3.b. This testing measures overall leakage across the PAL barrel from inside containment to outside. During thir process, the outer door was opened as required by the surveillance procedure, but due to a misinterpretation of Technical Specification requirements, individual door seal leakage across the outer door was not tested within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> after opening, per the requirements of Technical Specification 4.6.1.3.a. This condition was caused by personnel error on the part of the individual that coordinated the PAL testing. Corrective actions included counseling the involved individual.

On March 12, 1996, Engineering Surveillance Test (EST-222), Containment Building Personnel Air Lock (PAL)

Barrel Overall Test (EIIS Code NH-AL) was performed to satisfy the six month type-8 local leak rate testing (LLRT) required by Technical Specification 4.6.1.3.b. This testing measures overall leakage across the PAL barrel from inside containment to outside. During this process, the outer door was opened, as directed by the surveillance procedure, to place a pyrometer in the airlock chamber to monitor temperature. Following completion of EST-222, the PAL outer door was not tested for individual door seal leakage within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> as required by Technical Specification 4.6.1.3.a. This condition was identified on March 16, 1996, when the Operations control room staff, closing an associated Equipment Inoperable Record, questioned Engineering (LLRT) personnel about Technical Specification testing requirements. When this question was raised, an incorrect determination was made by the LLRT test coordinator that the overall barrel test satisfied the testing requirements for individual door seal leakage. This determination was also contrary to a precaution and limitation contained in EST-222, which states that the individual door seal leakage requirements of Technical Specification 4.6.1.3.a are not satisfied and that the PAL Door Seal LLRT Test (EST-219) shall be performed as required.

This condition constitutes a violation of Technical Specifications. The decision to not perform an individual door seal leakage test following an overall barrel test has also occurred previously. It has been a practice during the last 4 barrel tests (approximate 2 year period) for LLRT testing personnel to use the barrel overall test to meet the individual door seal test requirements. Prior to the recent four barrel overall tests, the PAL outer door was immediately tested following removal of the temperature monitoring pyrometer. This condition is reportable per 10CFR50.73(a)(2)(i)(B).

CAUSE:

This condition was caused by personnel error on the part of the individual coordinating the PAL LLRT testing.

Specifically, upon completion of EST-222, the LLRT test coordinator determined that individual door seal leakage testing on the outer door was not required since the overall test had been satisfactorily completed. This determination was based on past experience and an interpretation of design testing requirements, but was not cognitive. The individual incorrectly assumed that the PAL overall barrel test satisfied Technical Specification testing requirements and did not comply with the precaution and limitation contained in EST-222.

SAFETY SIGNIFICANCF<:

This Technical Specification violation resulted in no adverse safety consequences. The total PAL leak rate determined by EST-222 on March 12, 1996 was 332 sccm, which is below the Technical Specification individual door seal leakage acceptance criteria of 1688 seem.

PREVIOUS SIMILAR EVENTS:

There have been no previous similar events reported pertaining to PAL individual door seal leakage testing.

Corrective Actions Com leted:

1. The individual that incorrectly interpreted PAL testing requirements was counseled on March 18, 1996.
