NOTICE OF VIOLATION Centerior Service Company Docket No.
50-440 Perry Nuclear Power Plant License No.
NPF-58 As a result of the inspection conducted on October 29 through December 18, 1993, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (Enforcement Policy) the following violation was identified:
Technical Specification 3.3.2, Isolation Actuation Instrumentation, requires, in part, that with the number of operable channels less than required by the minimum operable channels per trip system take the action required by Table 3.3.2-1.
Table 3.3.2-1, requires in part, that with less than 1 operable delta flow - high channel for the reactor water cleanup isolation trip function the affected system isolation valves be closed within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.
Contrary to the above, on November 14 and November 15, 1993, there was less f
than one operable delta flow - high channel for the reactor water cleanup isolation "A" trip function for more than I hour and the affected system isolation valves were not closed within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.
l This is a Severity Level IV violation (Supplement 1).
l The inspect. ion showed that steps had been taken to correct the identified violation and to prevent recurrence.
Consequently, no reply to the violation is required and we have no further questions regarding this matter.
