SUMMARY Inspection on July 21-25, 1980 Areas Inspected
'
This routine, unannounced inspection involved 41 resident inspector-hours onsite in the areas of IE Bulletins (Unit 1); implementation of code requirements on socket weld size (Units 1 and 2); mechanical inspection of safety-related pipe hangers (Unit 1); safety-related piping observations of welding and non-welding work (Units I and 2); reactor coolant pressure boundary piping-observation of welding and non-welding work (Units 1 and 2).
Results Of the 5 areas inspected, no items ri noncompliance were identified in 2 areas; 2 items of noncompliance were found in 3 areas (Infraction - Undersize pipe to flange socket welds - Paragraph 6.a; Infraction - Welding material control -
Paragraph 7). No deviations were identified.

applicable licensee requirements.
In observing the examination the NRC inspector found the following noncompliance:
(1) The QC inspector failed to select the proper size criteria described in the applicable procedure (Procedure L 200 R10) and initially inspected the first weld to requirements for socket welds rather than for socket-welding flange welds.
In practice use of the wrong criteria could result in acceptance of undersize welds.
The QC inspector was shown the proper procedure requirements and subsequently inspected the selected welds to'the correct require-ments. The subject QC inspector inspects welds for both Units 1 and 2.
(2)
M e procedure (L200) appeared inadequate in that determination of
.ne specified minimum weld size required the craftsman who performed the weld and the QC inspector to make two computations. The procedure is applicable to weld inspection for Units 1 and 2.
(3) The five welds selected and inspected were all found to be under-size to the procedure requirements. The undersize welds were as follows:

Mechanical Inspection of Pipe Hangers (Unit 1)

The NRC inspector selected several previously accepted safety-related pipe hangers and requested that a licensee QC inspector reinspect the hangers to demonstrate their acceptability and the QC inspector's ability to properly perform the recuired examinations. The hangers /
examined were identified as 1AKC-34/0 (Component CoolinF dystem) and IRNV-37 (Chemical Volume Control System). The inspections observed by the NRC inspector included checks for proper materials, dimensions, identification, and clearances in accordance with procedures M-15 RII, M-51 R0, CP-115 R6, CP-309 R7, and CP-385 R-6.

-Safety Related Piping (Welding) - Observation of Work & Work Activities (Units 1 and 2)-
- The code applicable to the licensee's _ welding of safety-related piping is ASME Section III (74S74)..The NRC inspector observed welding in progress on a safety-related pipe weld and welding material contro1 ' activities for -
'conformance with cooe and procedural requirements. The weld,. identified 2NI67-3 (Unit 2 Safety Injection System Piping), was examined for proper identification / location, welding procedure, welder qualification, preheat and interpass. temperature, welding material, surface condition, and QC inspection entries on the weld record.
In examining the welding material for the above weld the NRC inspector found that while the welder's issue slip indicated the welder had been issued 1/8 inch E308-16 electrodes of Heat 8721 he actually had a mixtureHof those electrodes and 5/32 inch electrodes of heat 8617. The NRL inspector also examined weld material controls at the veld issue station (which issues welding material for RCPB and safety related pipe for both Units 1 and 2) for proper storage, issue records, and handling of returned material.
In this examination the NRC inspector found one E308 electrode mixed with oven stored E309 electrodes and one E308 electrode (Heat 8134) mixed with oven stored E308 electrodes of a different heat. The licensee's procedure H-3 R11 requires. segregation of electrodes by heat, size and classification. The examples of mixed elec-trodes cited above violate the procedural requirement and are considered to be in nons.apliance with Criterion V of 10 CFR 50 Appendix B. The licensee

was informed that this noncompliance would categorized an infraction, identi--

Also, the inspector observed safety-related 1 'JB system (Unit 1, Boron Recycle System) piping in sleeves for proper clearances in accordance with design variation Notice j
Serial No. 10216. Piping observed to be out tolerance was found to have been properly identified for disposition. An example was piping piece I hs-281.

The applicable code for reactor coolant pressure boundary (RCPB) piping is ASME Section III (74S74). The NRC inspector observed welding activities on two RCPB welds with the root passes completed:

The NRC inspector examined the welding for ' proper weld identification location, use of specified procedure, wald appearance, and welder identi-fication and qualification.
Welding material control activities were observed in the weld areas and at the weld issue station. As indicated in paragraph 7 above, inadequate welding material control was found at F e i
issue station.
The work site was observed to determine if an adequi.. e number of QC inspectors had been provided.

Reactor Coolant Pressure Boundary Piping - Observation of Work and Work Activities (Units 1 and 2)
The applicable code for RCPB piping is ASME Section III (74 S 74). The NRC inspector observed identification marking on RCPB pipe adjacent to welds INC 190-11 (Unit 1, Reactor Coolant System) and 2N I61-5 (Unit 2, Safety Injection System) for compliance with procedure H-4 R16. Also, the NRC inspector observed fitup of piping for weld INV 550-8 (Unit 1, Chemical Volume Control System) to assure compliance with cold spring requirements in licensee specification (NS-1205.02-01-000 Rev. 6).
