SUMMARY Inspection on May 27-29, 1980 Areas Inspected This routine, unannounced inspection involved 44 inspector-hours on site in the areas of electrical components and systems work and work activities; and electrical components and systems quality records.

Results Of the two areas inspected, no items of noncompliance or deviations were identified in one area; two items of noncompliance were found in one area (Infraction -

inadequate procedures for the inspection of safety-related electrical cables -

Paragraph 6; Infraction - construction electrical test program - Paragraph 6).

Exit Interview The inspection scope and findings were summarized on May 29, 1980 with those persons indicated in Paragraph 1 above. The items of noncompliance identified in paragraph 6 were discussed in depth.

Independent Inspect'on Effort The inspectors examined the installation of the diesel generator battery racks to assure that the installation was in accordance with ret;ulatory requirements for safety-related equipment. The inspectors observed welds on the upper-cross pieces of the stair-step battery racks and questioned the workmanship. The licensee is presently evaluating the adequacy of the welds to assure that they meet the seismic requirements. The battery racks were vendor supplied. An inspector followup item will be opened to review the seismic qualifications of the diesel generator battery racks. This item will be identified as inspector followup items 413/80-11-03 and 414/80-11-03, Review of Seismic Qualifications of Vendor Supplied Diesel Generator Battery Rack.

Motor Operated Valve Motor - 1RN148B The above equipment was examined to assure that the requirements of the PSAR and QAM were complied with in the areas of receiving inspection, storage, handling, identification, installation, construction procedures, nonconformance control, protection, separation and inspector qualification.

A bus intertie was identified between batteries 1 and 3.

This intertie is identified in the PSAR in paragraph 3.3.2.1.2 as a manual connection for use during battery maintenance.

Later research identified an intertie between batteries 2 and 4 also. The inspector questioned the control of the breakers during normal operation to assure independence of each battery.

l The licensee could not identify the control which would prevent inadvertant paralleling of the batteries. This will be an unresolved item identified

as 413-414/80-11-01, control of 125 VDC bus intertie.

The inspectors asked for the cable termination records for the Motor Operated Valve (MOV) motors, whereupon the licensee informed the inspectors that no electrical inspection had been performed. The inspectors gr.astioned this answer as the valves were energized. The inspectors examincd procedure M40 Revision 6, dated July 16, 1979 which states in paragraph 2 that it covers installation and documentatioa of all nuclear safety related cables.

Paragraph 4.3 of the procedure states, " Terminations shall be verified by signing the appropriate spaces on the termination card".

The licensee stated that procedure M40 is to be cancelled and replaced by a supplemental inspection instruction M41B serial 15, Revision 0, dated August 28, 1979.

The M41B serial 15 does not require inspection of the terminations and connections, however the PSAR in paragraph 8.3.1.1.6b states, "All interconnecting wiring and cabling will be inspected for proper installa-tion and connections". Duke Power Company's accepted Topical Report IA,

1.30 which endorses ANSI N45.2.4 which requires inspections for tightness of connections and fastenings and terminations. This is an infraction and will be identified as 413-414/80-11-02, Inadequate procedures for the inspec-tion of safety-related electrical cables.

The M41B serial 15 requires a sample of crimping tools to be checked with a G0/N0 GO gage, Construction Procedure CP-388 G0/NO-GO Gage Test, Revision 0, dated September 25, 1979 details the test. Neither procedure documents the results. CP-388 is used for the Thomas and Betts WT-145A tool. Proce-dure M40 notes that power and special terminations may use other tooling.

The licensee has identified no other crimping tool tests. Thomas and Betts recommend a monthly test of tooling. There is no periodic testing of all crimping tools to assure adequate connections. This item will be identified as inspector followup item 413-414/80-11-04, Calibration of crimping tools.

The licensee stated that prior to energization the craft make a circuit test and red line a drawing. This test is not covered by any procedure, is not documented and QC does not inspect this test. ANSI N45.2.4, Paragraph 5.2 requires that tests be performed in accordance with written test procedures.

This is an infraction and will be identified as 413-414/80-11-05, Construc-i tion electrical test program.

Electrical (Components and Systems II) - Review of Quality Records Th'e inspector examined the quality records for the electrical components identified as items a thru j in paragraph 6 above. The records were examined to assure that the requirements of the SAR and QAM were complied with in the areas of receipt inspection, material certification, storage, handling, identification and installation inrpection.

Within the areas examined, there were two items of noncompliance identified.
