INSPECTION REPORT NOS.. 50-369/89-31 AND 50-370/89-31 ' Thist befirs to the special, onsite inspection conducted by the Nuclear Regulatory ' ' l Commission's (NRC); Augmented Inspection Team (AIT) during the period September 7-11. ' ' 11989,-at the McGuire; Nuclear Station.-'The inspection included a review of the ' W 1 events related to the overpressurization of the Unit 2 Containment' Spray System

- The" AIT:conclu'ded that the overpressurization event 'was caused = by a deficient
- test procedure and ani operator error.
The test procedure did not. specify
- plant condition -limitations nor warn' the operators of the potential for over-
, ', L pressurization.
The operators ~ have not received -specific training in. the ' potential hazards ofxInter System LOCAs (ISLOCAs) and failed to recognize the f ' potential consequences of their actions.

On July 21, 1988, a 'similtc , ~ essurization event occurred on Unit 2'and the NRC issued a Notice . .of Viciatior for ani inadequate test procedure., The corrective actions in , 1responsento p t violation.were apparently too narrowly focused' to have precluded I' the~ similar event? of September 5,' 1989.
Although several procedure changes.
" h Lwere made to correct the perceived problem, the vulnerability of the containment j ? spray: system to overpressurization was not recognized.
The staff elected not ' to, hold : an~ enforcement conference 'as allowed by the Enforcement Policy for - lsimilar Severity. Level IV violations because of the minimal safety significance . Lof'the' events. ' However, your response to the violation should include detailed L 'i information. delineating the scope of corrective actions to address this problem.

Furthermore, this event was not reported to the NRC.
The NRC believes that , this. event should have been reported within one hour as required by L10 CFR 50.72 b(1)(ii)(A) and (B).
Also, the NRC resident inspectors were not informed of this event and only became aware of it through their own inspection lj

.The: responses directed by this letter and its enclosure are not subject to the clearance procedures of, the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
