Licensee Action on Previous Insoection Findings Recovery Operations Organization Because of the March 28, 1979, accident, the licensee's organization

had several structural and functicnal changes.

By letter to Metropolitan Edison dated September 15, 1980, the NRC approved the General Public Utilities Nuclear Group (GPUNG) organization that would manage the cleanup efforts at Tril-2.

During this review it was noted that Administrative Procedure (AP) 1009, Revision 3, October 28, 1977, Station Organization and Chain of Command, was not updated to reflect the current organization structure, responsibilities and lines of authority.

The licensee representative stated that action to update this procedure and other administrative control procedures was started prior to NRC approval of GPUNG.

This procedure is expected to be updated by the end of December 19S0.

It was also noted that AP 1009 was not specific in terms of addressing personnel outside. the operations and maintenance departments.

This area (320/ 79-13-12) remains open pending the upgrading of administrative controls for organization structure, delination of responsibilities and lines of authority.

Facility Procedures Submitted for Approval Facility procedures and subsequent revisions, required to be submitted for approval to the NRC as required by the Order of February 11,1980, ( Attachment 1 - proposed Technical Speci fi-cation 6.8.2), were reviewed by the NRC TMI Program Office staff.

These procedures address the Recovery Operations Plan Implementation (Surveillance Procedures) and Recovery " ode Implementation (Operating Procedures).

Detailed review of selected procedures included both health physics and operations aspects with consideration of the following:

(1) the procedures, when implemented, would not degrade the containment of radioactive material, jeopardize core cooling,

or result in excessive personnel exposures; (2)

the technical content of the procedures are adequate to perform the intended evolution.

Composite staff comments on procedures were forwarded to the licensee.

Licensee resolution of these comments was acceptable.

4.

Plant Operations Review a.

On a periodic basis the resident inspector obtained information on plant conditions, reviewed selected plant parameters for abnormal trends, ascertained plant status from maintenance /

modification viewpoint, and assessed logkeeping practices in accordance with administrative controls.

During the review the resident inspector made random visits to the control room during regular and backshif t hours, discussed operations with control room personnel, reviewed selected control room logs and records and observed selected licensee plan of the day meetings.

In addition, a tour of the auxiliary and fuel handling buildings was conducted on October 18, 1980, specifically to review fire protection measures in these areas.

During the review of fire protection measures in the auxiliary and fuel handling buildings, the following observations were made as of October 16, 1980:

--

Certain fire extinguishers were not located at designated stations.

For example, station 13 and 14 in the auxiliary building had no mounted extinguisher nor was an extinguisher in the area.

All fire extinguishers reviewed (approximately 10) did

-

not have documentation of monthly checks for August or September 1980 or both on the attacned tags.

Two extinguishers had outdated yearly tags.

An extinguisher,

-

near the H2 Purge area on the 328' level of the auxiliary building had a 1979 tag and station 17 in the same area had a 1977 tag.

It appeared annual checks were not done on the two extinguishers.

--

Certain extinguishers did not have the station numbers marked on them.

This occurred for an extinguisher at station 11 and for an extinguisher in the 2-llE room of auxiliary building.

The licensee representative indicated that similar findings were noted in a licensee conducted fire inspection of June 1980 with a report dated June 30, 1980, (Memo, TMI-II-R-140 - Item No. 168).

As a result of this finding, on October 20-21, 1980, a survey of all auxiliary and fuel handling building extinguishers, was conducted by the licensee to correct specific (previously idcatified) extinguisher problems and review for similar problems. The licensee representative reported the above types of discrepancies were corrected for the subject extinguishers.

This area will be reviewed further by the NRC (320/80-15-01).

In addition, during the above noted tour a significant amount of boron encrustation was observed on the floor in the area of the Standby Pressure Control (SPC) System (New Fuel Storage Pool - 347' level of the fuel handling building).

The licensee representative indicated that this was previously identified and stated that action would be initiatei to give increased e

attention to this area.

The inspector had no further questions in this area.

Progress in this area will be reviewed on subsequent routine inspections.

5.

Health Physics and Environmental _ Inspection and_ Review a.

Plant Tours Shif t inspectors completed a general plant inspection tour daily.

These inspections included all control points and selected radiologically controlled areas.

Observations included:

--

Access control to radiologically controlled areas; Adherence to Radiation Work Permit (RWP) requirements;

--

Proper use of respiratory protection equipment;

--

Adherence to Health Physics and Operating Proccdures;

--

Use of survey meters including personnel frisking techniques;

--

Cleanliness and housekeeping conditions; and, Fire Protection measures,

Peasurement Verifications Peasurements were independently obtained by the NRC onsite l

inspectors to verify the quality of licensee performance in l

the following selected areas:

Radioactive material shipping;

Radiological control, radiation and contamination surveys;

and,

Onsite environmental air samples.

No items of noncompliance were identified.

6.

Radiation Protection Prooram Review During the inspection period a special inspection of licensee commitments made as a result of the NRC Special Panel Review on the Unit 2 Radiation Protection Program (report dated December 7,1979)

was conducted.

Findings in this area were addressed in Office of Inspection and Enforcement Report No. 50-320/80-16.

Exit Interv_i_ew i-On October 22, 1980, the resident inspectors met with licensee representatives denoted in paragraph 1 to summarize the inspection scope and findings.
