Areas Investigated: Unannounced investigation of an allegation from a TMI employee wno questioned the recent rehiring of a fomer TMI employee.

It was alleged that the rehired employee made an unauthorized entry onto Three Mile Island on July 13,1979 at 11:10 PM, entered the Unit #2 pumphouse, and tripped a Nuclear River Pump.

BACKGROUND Reason for Investigation On March 1, 1980, a Metropolitan Edison Employee contacted an NRC Inspector and related, in confidence, his concern about the rehiring of a fomer Metropolitan Edison employee. He had heard from another employee that this rehired employee had allegedly used a boat to gain access to the Three Mile Island site on the evening of July 13, 1979. At that time, the rehired employee was not in the employ of the Metropolitan Edison Company.

It was further alleged that this rehired employee reportably tripped a Nuclear River Pump (NRP-1A).

It was further alleged that this rehired employee may have been armed when this ex-cursion took place.

An investigation into this allegation was initiated at the Three Mile Island Nuclear Generating Station by the NRC, Region I on March 5,1980.

_ II. StM4ARY OF FINDINGS

A.

~ Allegation and Investigation Findings

'

The NRC representatives' investigated this Alegation which was initially repcrted in confidence to an on-site NRC Inspector. The allegation and NRC findings are as follows:

1.

Allegation

,

On March 1, 1980, a Metropolitan Edison Company employee Individual

"A" at Three Mile Island, who requested confidentiality, advised that l

a fomer and recently rehired employee, Individual

"G" is alleged to have made coments to a current Metropolitan Edison employee, Individual i

"E", that on July 13, 1979 the fomer employee, Individual

"G" entered the Three Mile Island Nuclear Reactor site by boat, entered the pump-

I house and tripped a Nulsar River Pump, (NRP-1A).

It was further alleged that Individual

"G" said he was amed at _the time. The alleger,

Individual "A" stated he became concerned when he discovered that the i

former employee, Individual "G" was recently rehired.

Findings The NRC investigation found that the NRP-1A Pump had been tripped for

no apparent reason at 11:10 PM, July 13,-1980 and that this pump trip

- was unique since no mechanical or electrical reason was identified as i

a cause..Furthemore, the NRC Investigation revealed that the subject did in fact tell at least three persons that he had been responsible

for the excursion and/or subsequent pump trip. The subject denied to

,

the investigators that he had actually entered TMI during the time-

frame in question but admitted that he had fabricated the story in-

jest and had told the story to friends.

reason such as was the case. The investigators also determined that the pump can te tripped, from within the pumphouse, by an individual turning

either a creaker relay switch and/or by depressing a stop switch next to the pump. The investigators also determined that since the intake struc-

ture (pumphouse) was not located in either a Protected and/or Vital Area as

defined in the licensee's approved security plan and because of the absence

{

of electronic and/or security f>;rce obligated surveillance; it would have i

been possible to have penetrated the site and intake structure on the

evening of July 13, 1979 and to have departed the site without being de-tected.

i III. DETAILS A.

Introduction This investigation was initiated as a result of an allegation made to an on-site NRC, Region I Inspector on March 1, 1980.

B.

Scope of Investigation This investigation was limited to the allegation that an unauthorized individual may have gained illegal entry to the TMI Unit #2 Intake Struc-ture and may have tripped a Nuclear River Pump - NRP-1A on the evening of July 13, 1979.

This investigation included an examination of pertinent documents such as Unit #2 Control Room Operator and Shift Foreman logs, maintenance history of Nuclear River Pumps, security incidrac report files, employment records and discussions, interviews and con'. acts with site personnel and one person not in the employ of the licensee. Such interviews and related statements were ebtained both on the site and at other locations within the adjoinirig comunity.

Site Investigation The investigation was conducted at the TMI Site on March S-7 and March 11-

14, 1980. This investigation included (1) identifyin personnel deemed to have knowledge of this event, (2)g and interviewing confirming the status of Physical Security Systems with regard to the Unit #2 intake structure on the evening of July 13,1979,(3) confirmation of a Nuclear River Pump Trip (NRP-1A) on July 13, 1979, physical examination and a historical review of pump trip frequency and causes. Results were as follows:

(1)- Individual "A" - executed a sworn and signed statement. This Met-Ed employee admitted that he had heard of the allegation in the fall of 1979 from another plant employee, Individual "E" and that he became-acutely concerned upon learning that Met-Ed had rehired this former employee, Individual "G".

Individual "B" - executed a signed statement. This Met-Ed employee stated that he had first heard the allegation on or about February 27, 1980 from another Met-Ed employee, Individual "A".

This suployee,

Individual "B" emphasized that it was highly unlikely for a Nuclear River Pump such as NRP-1A to trip off for no apparent reason.

Individual "C" - executed a sworn and signed statement. This Met-Ed employee heard of the allega' dan from a. non-Met-Ed employee, Indivi-

dual "D" who had reportably neard the allegation being discussed by some "other boaters" (not further identified) in July,1979.

"G" allegedly tripped the pump (NRP-1A) switching a breaker switch off and then to the reset mode and how Individual "G" allegedly gained access to the TMI Screenwell House

Individual "0" - executed a sworn and signed statement. This non-Met-Ed employee heard the allegation directly from Individual "G"' at a connunity boat club location on or about the 2nd/3rd week of July 1979. The rehired Met-Ed employee, Individual

"G" described to Indivi-

dual "D" how he sneaked into the TMI Power Plant on a Friday or

,

I Saturday night.

(Note: NRP-1A tripped at 11:10 PM, Friday, July 13, 1979 for "no apparent reason").

Individual "E" - executed a sworn and signed statement. This Met-Ed employee first heard of the allegation in the fall of 1979 directly

'

from Individual "G" at a local bar in Middletown, Pennsylvania.

"G" reportably asked Individual "E" if there was a pump trip at the plant? This Met-ed employee responded that a Nuclear River Pump (NRP-lA) had in fact tripped for "no apparent reason." The recently rehired employee Individual "G" then stated' that he Indivi-dual "G" had come on the site and tripped a pump. This Met-Ed employee,

Individual "E"'further stated that Individual "G" had mentioned both a

'

gun and a boat during the discussion but could not recall in what context. This Met-Ed employee, Individual "E" further stated that Individual "G" had been in touch with him since this investigation was initiated and that Individual "G" instructed him Individual "E" to inform the investigators of a rumor that he Individual "G" was involved i

in' the March 28, 1979 TMI accident. The Met-Ed employee's impression was that Individual "G" wanted him, Individual "E",to create confusion by bringing up the accident.

Individual "F" executed a sworn and signed statement. This Met-Ed i

employee first heard of the allegation the week of February 25, 1980 from another Met-Ed' employee, Individual "B" who had heard of the allegation from another Met-Ed employee, Individual "A".

"G" brushed the question off as a joke.

The recently rehired employee, i

Individual "G" denied any knowledge of the allegation. This Met-Ed

-

employee further confronted Individual "E" with the allegation and was told by Individual "E" that in fact Individual

"G" had told Individual

"E" that he Individual "G" had snuck into the plant and had tripped

-

the Nuclear River Pump.

Individual ~ "G" - refused to execute a sworn or written statement.

Individual "G" worked for the Met-Ed Company on two previous occasions prior to having been recently rehired in 1980. The investigators apprised Individual

"G" of the allegation whereupon Individual

"G"

stated the following according to investigator's notes and recollection:

Individual "G" and Individual "E" were in a bar drinking and talking about the clant in general in August, 1979 and Individual

"E" told him Individual "G" that a Nuclear River Pump had tripped for some unknown

reasons. He Individual

"G" responded by saying to Individual "E" that since he Individual'"G" had bean blamed for everything else - that they'(Met-Ed) may as well blame him Individual

"G" for tripping the Nuclear River Pump. He, Individual

"G" said he Individual

"G" was drinking at the time of the conversation and that he, Individual

"G" felt that he, Individual "G" was being blamed by people (not further

identified) for causing the March 28, 1979 accident and for that

',

reason he, Individual "G" decided to say he, Individual

"G" had also tripped the Nuclear River Pump in order to start another rumor. When asked about the nature of the rumors and sources by the investigators, he, Individual"G"wasunabletoprogeanyspecificsand/orto

identify any sources of the rumors.

y When reconfronted with the allegation by the investigators, he Indivi =

dual "G" denied any involvement and said, "he, Individual "G" was only shooting his mouth off." He, Individual

"G" admitted promoting the

'

runor by telling people that he Individual

"G" tripped the pump.

It should be noted that he, Individual

"G" repeatably expressed his dislike for the Met-Ed organization.

Individual "G" initially agreed to execute a sworn statement denying

,

that he, Individual "G" tripped the Nuclear River Pump but declined to

,

provide a statement stating that he, would be unable to cite specific i

infonnation as to dates and places because of his excessive drinking i

habits.

Investigators Coment: During the course of this investigation, about

!

95% of personnel contacted and/or interviewed said that Individual

"G" did not drink to excess and/or frequently,

'

"G" was

attempting to direct the interview theme away from the basic allegation.

(2) Status of Physical Security Systems Effecting the Protection of the Unit #2 Intake Structure, July 13, 1980 Discussion with TMI operations and security staff officials revealed that as a result of the March 28th accident, installation of the 10

CFR 73.55 Upgrade Systems planned for the Screenwell House Facility (Intake Structures #1 and #2), were in a deferred mode on July 13, 1979. Thus, surveillance of this area was limited to mobile security patrols (approximately every 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />) and 1 walking patrol. These patrols were responsible for patrolling the periphery of the adjacent i

protected area and vital area doors within the adjacent protected area. Observation and surveillance of the intake structure facilities was, for the most part, based on the frequency of maintenance checks performed on equipment in the intake structure by auxiliary operations personnel, estimated at two times per 8-hour shift. Although the structures were surrounded and enclosed by a chain link fence including an access gate; the keys were not rigidly controlled sint' the equip-ment had not been classifed as " Vital" in the Approved Security Plan.

A check of the Security Departnents " Security Incident Files' revealea that the gate was found unlocked and open on July 14, 1979 (one evening afte-the July 13, NRP-1A trip) due to probable personnel error. The intak e structure equipment (roll-up) and personnel entrance doors were supposed to be kept locked but there was no rigid key control procedure to give this assurance.

Lighting arouni and within the screenwell facility was operational and adequate to idnntify an intruder. There was no operational CCTV cameras and/or,1erimeter intrusion detection system on July 13, 1979.

'

Therefore, penetration of the area and intake structures could have been accomplished with relative ease on the evening of July 13, 1979,

particularly if an intruder knew the area well and approached the

'

structure from the Susquehanna River side of the plant by boat.

Description of' Nuclear River Punp'NRP-1A and Maintenance History of Trips for Unexplained Reasons Nuclear River Pump, NRP-1A, is one of 4 Nuclear Service River Water System Pumps which provide cooling water for various safety related components.

Included are the Emergency Diesel Generator lubricating oil and air coolers, the nuclear services coolerr, intermediate coolers, which provide cooling for the Decay Heat Removal Service Coolers, and

Reactor Building nomal cooling coils. Two such pumps must be main-

tained on-the line at all times thus there are a total of 4 to insure

the required back-up redundancy and are part of safety related equip-ment. Discussions with Met-Ed officials revealed that these pumps can

!

be tripped remotely from the Control Room and/or by tripping a breaker switch and/or by depressing the switch on/off button directly adjacent to the pump located in the intake structure.

Reproductions of photo-graphs taken during this investigation are provided in Section III of this report.

Further discussions with the Maintenance Supervisor at Met-Ed revealed

'

that it is a rare event for a Nuclear River Pump to trip for "no apparent reason" i.e. no mechanical and/or electrical cause.

The NRP-1A pump trip on July 13, 1979 is thus unexplainable based on at least a 15 month review of Nuclear River Pumps maintenance history.

It was pointed out by maintenance personnel that 2 years ago, loss or

reduction of a water level input would occasionally cause a trip but

this problem has since been long corrected.

Further discussions on how unexplained Nuclear River Pump trips are handled revealed that

,

with such an occurrence such as the NRP-1A trip on July 13 at 11:10 PM, it is normal practice to send either an operator or an auxiliary

'

'

operator to the pump ta actempt to detemine the problem. The problem is nomally identific.d as mechanical / electrical reason i.e. short,

wire cut, breaker all bearing leak or some obvious cause which is

'

reported h a work order to maintenance. Since on July 13, 1979 such cursory investigation revealed no obvious problem; the pump was re-started without difficulty and thus no work order was initiated to i

the maintenance activity. There was no thought given to the possibility

'

that perhaps an individual may have caused the trip and the matter was dropped.

It was also learned that the trip of NRP-1A %s of marginal

-

safety consequcoce since one of the backup pumps came on the line

immediately.

Copies of both the Contol Room Operator and Shift Foreman logs recording the NRP-1A Pump Trip at 2310 hrs, July 13, 1979 are included in Section III of this report.

The investigators confirmed by recorded documentation and discussions with personnel that Nuclear River Pump NRP-1A did trip off for no apparent reason at 2310 hrs, July 13, 1979 and that the subject of this investiga-tion, Individual

"G" had told at least three individuals that he (Subject)

had entered onto the site illegally on or about the July 79 timeframe and

that-the subject told two of these three individuals directly that he (Subject) had been responsible for causing the pump trip and that he (Sub-ject) had perpetrated this act to demonstrate a weakness in the site security program. The investigators further confirmed that because of the TMI March 28, 1979 accident and a corresponding spin off deferral of associated

'

systems security upgrade projects; undetected penetration of the intake structure, which was not designated either as a Protected or Vital Area, in the approved security plan, could have been accomplished with a high level of probability. The investigators also confirmed that Nuclear River Pumps such as NRP-1A do not have a history of tripping off for no apparent reason and that this trip on 2310 hrs, July 13, 1979 was considered extremely i;nusual because of no identifiable mechanical and/or electrical failure.

Management Meetings The investigators met with NRC and licensee representatives (denoted in Paragraphs C-1 and C-2 at the conclusion of the investigation on March 4, 1980.

The investigators summarized the scope and findings of the investigation as of the time of this meeting.
