OnJune8,1990,RG&EnotifiedtheNRCunderthecriterionof10CFR550.72,ofaconditionoutsidethedesignbasisoftheplantassociatedwithAOV745,thecomponentcoolingwaterisolationreturnvalvefromtheexcessletdownheatexchanger.ThisnotificationwasmadeasaresultofinformationcontainedinTable3.6-1oftheGinnaTechnicalSpecificationsandUFSARTable6.2-13,whichindicatedthatAOV745shouldreceiveaContainmentIsolationSignal(CIS).However,elementarywiringdrawingsdidnotshowthisvalveasreceivingsuchasignal.Subsequenttothisnotification,RG&EdeterminedthatinformationcontainedonTechnicalSpecificationTable3.6-1forMOV749AandMOV749B(componentcoolingwaterisolationvalvestothereactorcoolantpumps)wasalsopotentiallymisleadingbasedonthefactthatthesevalvesdonotreceiveaCISbutareshownwithamaximumisolationtimeof60seconds.Consequently,RG&EhasreviewedthebackgroundinformationavailablewithrespecttoAOV745,MOV749A,andMOV749B.AllthreevalveswerereviewedbytheNRCduringimplementationoftheTMILessonsLearnedrecommendationsandtheSystematicEvaluationProgram.Penetration124awasevaluatedinthesetwoprogramsassumingthatitwasnonessential,andthatAOV745wasanormallyopenvalvewhichdidnotreceiveaCISbutwasclosedpost-LOCA.Meanwhile,Penetrations127(MOV749A)and128(MOV749B)wereconsideredessentialpenetrationswithnormallyopenedvalvesthatalsodidnotreceiveaCISbutremainopenpost-LOCA.InaNovember19,1979lettertotheNRCdiscussingTMILessonsLearneptopics,RG&EstatedthatPenetration124awas"nonesbential"whilePenetrations127and128were"essential".InpreviouscorrespondencefromRG&Eonthissubject(datedJune22,1979),atablewasprovidedwhichlistedallvalvesreceivingaCIS;neitherAOV745,MOV749A,norMOV749Bwerelistedonthistable.9007l70340900709PDRAGOCK05000244pplleap

/ep44I InTable1oftheFinalEvaluationReportforSEPTopicVI-4,datedApril12,1982,itisindicatedthattheNRCassumedthatAOV745wasnormallyopen,butclosedpost-LOCA.However,thereisnoinformationlistedbeneaththe"Actuation"columnforthisvalvewhichwouldindicatehowthevalvewasexpectedtoclose.ThistablealsoshowsMOV749AandMOV749Basnormallyopenvalveswhichremainopenpost-LOCA.Thesevalvesareshownas"remotemanual"under"Actuation".Inaddition,drawingsandtheoriginalGinnaFSARasusedbytheNRCduringtheSEPdidnotshowthattherewasaCIStothesethreevalves.Therefore,theNRChadevaluatedAOV745,MOV749AandMOV749BintheircurrentconfigurationsduringboththeTMILessonsLearnedandSystematicEvaluationPrograms,andfoundthemtobeacceptable.Sincetherewasnoconflictingsupportingdesignbasisinformation(e.g.,UFSAR,elementarydrawings)showingthatMOV749AandMOV749BreceiveaCIS,thesevalveswerenotdeclaredinoperablewhenitwasdeterminedthattheydidnotreceiveaCIS,ashadbeendonewithAOV745.Basedonourreviewofthissituation,RG&Einterpretsthe60secondmaximumisolationtimeaslistedinTechnicalSpecificationTable3.6-1tobevalveclosuretimefollowingademandtoclose.ThisinterpretationisconsistentwithStandardReviewPlan6.2.4,sectionII(n)andhasbeentransmittedtotheGinnaStationoperationstaff.'Therefore,nochangesarerequiredwithrespecttoMOV749AandMOV749B.Penetration124aisaclosedsysteminsidecontainmentwhichisseismicandmissile-protected.ThecurrentGDC57onlyrequiresamanualorremote-manualvalveoutsidecontainmentforthisconfiguration.Thesafetyfunction(i.e.,containmentisolation)ofthispenetrationismetinthecurrentconfiguration,evenwithAOV745normallyopen.RG&EhasthusconcludedthatAOV745doesnotrequireaCIStobeconsideredoperablesincethecontainmentisolationfunctionassociatedwithPenetration124aisaccomplishedbytheclosedsysteminsidecontainmentandAOV745(withoutaCIS)outsidecontainment.Therefore,thecriteriaof10CFR550.73arenotapplicabletothediscoverythatAOV745doesnotreceiveaCIS.However,becauseourresponsetoitem2.1.4.3intheabovecitedNovember19,1979letterindicatedthatwewouldprovideaCIStovalvesinnon-essentialpenetrations,eventhoughsuchasignalgoesbeyondtherequirementsofGDC57,RG&EcommitstoinstallsuchasignaltoAOV745bytheendofthe1992refuelingoutage.Inthemeantime,inordertoprovideanotherlevelofassuranceofcontainmentisolation,RG&EisplanningtomodifytheEmergencyProcedurestodirecttheoperatorstoremote-manuallyclosethisvalvefromtheControlRoomatanappropriatetimefollowingreceiptofaCIS(e.g.,followingimmediateactionsofE-0).TheadditionofthisprocedurestepwillbeaccomplishedbyAugust15,1990.
