The attached Licensee Event Report LER 1999-004 is submitted in accordance with 10 CFR 50.73, Licensee Event Report System, item (a) (2) (ii) (A), which requires a report of, "Any event or condition that resulted in the condition of the nuclear power plant, including its principal safety barriers, being seriously degraded; or that resulted in the nuclear power plant being: (A)

In an unanalyzed condition that significantly compromised plant safety", and in accordance with 10 CFR 21, "Reporting of Defects and Noncompliances" ~ A further assessment will be provided in a supplement to this LER which will be submitted by June 30, 1999.
