ENCLOSURE1COMMITMENTTRACKINGPAGE1OF4INTRODUCTIONDetail6.9ofInspectionReport50-244/90-09concludedthatthecommitmenttrackingsystematGinnamaynotbefullyeffectivebecausetherearenoapparentplansforaddressingcertainitems.ReportDetail7.1identifiedanapparentprolongedfailuretomeetacommitment(tomaintaintheexcessletdownheatexchangercomponentcoolingwaterreturnlinecontainmentisolationvalveasnormallyclosed).TheInspectionReportcoverletterrequestedthatweevaluatetheabovematters,thatweinformyouoftheassessmentofourabilitytomeetandadheretosimilarsafetycommitments,andofanyplannedcorrectiveactions.BothInspectionReportDetails6.9and7.1addresstheimplementationandmaintenanceofcommitments.However,theconcernsidentifiedinReportDetail6.9applytocommitmentswhichtypicallyrequireaone-timeactiontocomplete(e.g.,NRCViolations,UnresolvedItems,Deviations).ReportDetail7.1appliestodesigncommitmentswhichrequirecontinualcognizanceandimplementation(e.g.,operatingAOV-745asnormallyclosed).Eachoftheabovecommitmentcategoriesrequireuniqueimplementationandmaintenanceprocesses.Accordingly,ourcorrectiveactionswillincludeseparatemethodsfortrackingeachoftheabovecategoriesofcommitments.Webelievethatbothofourmethodswillprovideuswithahigher.levelofassurancethatcommitmentsarebeingmaintainedaswellasprovidingamoreefficientmeansoftrackingcommitment-relatedinformation.Bothofourmethodsarediscussedbelow.ThesectiontitledCommitmentandActionTrackinSstemoutlinesourapproachtotracktheone-timeactioncommitmentsthatarediscussedinReportDetail6.9.ThesecondsectiontitledDesicenBasisDocumentsoutlinesourapproachtotrackingdesigncommitmentsdiscussedinReportDetail7.1.RG&E'sCommitmentandActionTrackingSystem(CATS)whichhasbeenunderdevelopmentsince1989,isnotthetrackingsystemwhichwasreviewedandidentifiedinReport.Detail6.9.RG&Ehasdevelopedtheplans,isdevelopingthemainframesoftware,andwillprovidethenecessaryorganizationwhichwillallowfortheintegrationofanumberofcurrenttrackingsystems,oneofwhichwasreviewedinReportDetail6.9,intoasinglesystemforregulatoryandothertypesofcommitments.

ENCLOSURE1(Continued)COMMITMENTANDACTIONTRACKINGSYSTEMCATSPAGE2OF4RG&Ehaspreviouslyrecognizedthestatedshortcomingswithourcurrentcommitmenttrackingmethods.ToaddressthisissueandconcernsnotedinInspectionReports88-15and89-05,aCommitmentTrackingAssessmentwascompletedinJanuary,1989.Subsequenttotheassessment,theRG&ECommitmentandActionTrackingSystem(CATS)ProjectPlanwasdevelopedandapprovedbyRG&E'sSeniorManagementinOctober,1989.Developmentofourupgradedtrackingsystemisprogressing.Asnotedat.theMarch6,1990NRCRegionI-RG&EManagementMeeting,theanticipatedimplementationscheduleforthetrackingsystemisMay29,1991(ReferenceRG&EConfigurationManagementLevelIScheduleincludedinthemeetinghandoutswhichareattachedtoInspectionReport50-244/90-03,datedApril18,1990).ThefollowingprovidesabriefdescriptionofsomeofthefeaturesoftheCommitmentandActionTrackingSystem(CATS):COMPUTERHARDWARE/SOFTWARECATSwillresideonourDataGeneralmainframecomputer.Theuseofthemainframewillallowaccesstoup-to-datecommitmentdatabybothplantandcorporateNuclearDivisionpersonnel.Theupgradedtrackingsystemwillsignificantlyexpandthetypesofdataelementsbeingtracked.Dataelementsinclude,butarenotlimitedto:responsibledepartmentandindividual;documentinformationsuchassourceofthecommitment,relateddocuments,andimplementing/closuredocuments;duedateandpriority;RG&Eandagencyclosuredates;status;agency;cause;plantsystems/structures;keywords;relatedcommitments;summaryanddetailedtextofthecommitment;andcomments.Eachcommitmentwillalsoincludeanactivitiessectionwhichwillallowfordevelopmentofactionplanswherenecessary.Theactivitiessectionincludes:activitynumber;departmentandindividual;descriptionofactivity;targetcompletiondateandactualcompletiondate;andactivitystatus.ThesoftwaredesignincludesmenustoallowforvarioustypesofreportstobeeasilyrequestedbyNuclearDivisionpersonnel.UserManualswillbedevelopedtoaidpersonnelintheuseofthesystem.TheDetailedSoftwareDesignfortheCATSiscurrentlyunderdevelopment.

ENCLOSURE1(Continued)PAGE,3OF4DATABASEOurestablishedgoalistoprovideanintegrateddatabaseofpotentiallysafetysignificantissueswhichoriginatefromvarioussources.Atthetimethesystemgoesoperational,thedatabasewi'll,ataminimum,consistofthoseopencommitmentswhichoriginatedincorrespondencetoorfromtheNRC.Inourcurrentplans,weexpecttoexpandthedatabasetoincludeothercommitments/actionitemsfromdocumentssuchasCorrectiveActionReports,IdentifiedDeficiencyReports,NonconformanceReports,ManagementActionItems,otherfederal,state,andlocalregulatoryagencycorrespondence,INPOEvaluations,andINPOgenericindustrycorrespondence(e.g.,SOERs,SERs,etc.)TheextentoftheitemstobeaddressedbyCATShasnotyetbeenfinalized.Toallowforthebuildingofacommitmentdatabaseconcurrentwiththedevelopmentofthemainframesoftware,aPCbasedtrackingsystemwasdevelopedintheNuclearSafetyandLicensingGroupasaninterimmeasureuntilthemainframesystemimplementation.Reportsgeneratedfromthisinterimsystemhavetheessentialdataelementsandappearancethatreportsgeneratedfromthemainframewillhave,withtheexceptionthattheactivitiessectiondiscussedinSection1abovewasexcluded.Currently,thePCdatabaseincludescommitments/actionitemswhichoriginatedincorrespondencetoandfromtheNRCdatedsubsequenttoJanuary1,1990.Thedatabasealsoincludesanumberofcommitmentswhichoriginatedpriorto1990yetremainedopen.Weareconductingadditionalreviewsofourcurrenttrackingsystemstoidentifypre-1990commitmentswhichremainopen.ThesereviewsshouldassurethatopenNRCissuesareidentifiedandincludedinthedatabaseatthetimethemainframesystembecomesoperational.Itisourintenttoretainclosedcommitmentsinthedatabasetofacilitatefuturehistoricalresearchofissues.ThedatabasethatexistsonthePCwillbeincorporatedintothemainframecomputersystematthetimeofitsimplementation.

ENCLOSURE1(Continued)PAGE4OF43~MANAGEMENTCONTROLSTheCommitmentandActionTrackingSystemwillprovideassurancetomanagementthatsafetysignificantissuesarebeingaddressed.Inadditiontotrackingthecommitment/actionitems,thesystemwillallowactionplanswithappropriatetargetcompletiondatestobeestablished.Periodicreportstomanagementandlinepersonnelshowingissuesoverdueandcomingdueareplanned.Administrativecontrolsandimplementingproceduresarescheduledtobedevelopedpriortofullsystemimplementation.Theseadministrativecontrolswillestablishresponsibilitiesforthemaintenanceandimplementationofcommitments.SufficientprogresshasbeenmadeonourdevelopmenttoallowustostartthedistributionofthesignificantcommitmentreportsfromtheinterimPCsystem.Atthistime,thesereportsarebeingusedtosupplementinformationfromothersystemsandprovideahigherlevelofconfidencethatcommitmentsarebeingmet.DESIGNBASISDOCUMENTSAsnotedattheMarch6,1990NRCRegionI-RG&EManagementMeeting,theRG&EConfigurationManagementLevelIScheduleshowsaJune16,1990startdatefortheDesignBasisDocuments(DBDs)andacompletiondateofNovember21,1994.TheDesignBasisDocuments(DBDs)currentlybeingdevelopedforusebyEngineeringandGinnaStationpersonnelwillcontainorreferencetheDesignBasisandLicensingcommitmentsapplicabletothestructures,systemsandcomponentscontainedwithinthem.TheDBDswillbecontrolledsothattheywillaccuratelyreflecttheup-to-dateDesignBasisandcommitments.TheywillprovidetherequiredreferencedocumentsforEngineeringandGinnaStationpersonneltoensurethatproposedmodificationsareproperlyreviewedandthattheDesignBasisandongoingcommitmentsareproperlymaintainedatGinnaStation.ThiswillreducetheprobabilityoffutureoccurrencessuchastheimproperlypositionedvalvedescribedinSection7.1oftheinspectionreport.AspreviouslynotedinNRCcorrespondenceandconversations,theDBDeffortarecentlyinitiateddocumentationprogram.Itwillbeperformedinasystematicfashion,withthemostimportantsystemsandtopicsreceivinghigherpriority.

ENCLOSURE2PAGE1OF1VIOLATION50-244/87-10-04DEVELOPMENTANDIMPLEMENTATIONOFANEFFECTIVESHELF-LIFEPROGRAMAsidentifiedinDetail6.6ofNRCInspectionReport50-22/90-09,theProcurementEngineeringGrouphasbeenassignedthetaskofdevelopment/implementationofacomprehensiveshelf-lifeprogramforitemsmaintainedintheGinnaStationWarehou'se.Developmentofthecomputersoftwarewhichwillbeusedtoidentify,trackandcontrolitemswithlimitedshelf-lifewasbegunJune15,1990.Basedontheprogressthusfar,completiondateforthisphaseisOctober1,1990.Developmentoftheimplementingproceduresfortheshelf-lifeprogramwasbegunJuly23,1990,withanestimatedcompletiondateofOctober1,1990.Thedecisionwasmadetodevelopthesoftwarefirstandtheprocedureswouldthenbedeveloped.Basedontheimplementationdatesidentifiedabove,theoverallshelf-lifeprogramwillbeavailableforimplementationbyProcurementEngineeringonOctober1,1990.Atthat.time,allprocureditemswillstarttobeprocessedthroughtheshelf-lifeprogramtoassurethatitemswithlimitedshelf-lifearecontainedinthecontrollingprogram.Previouslyprocureditemswillcontinuetobehandledinaccordancewithourinterimpolicyof"Last-In,First-Out"(LIFO).TheProcurementEngineeringGroupwillalsoperformareviewofpreviouslyprocureditemsmaintainedinthewarehouse.Thisprogramwillbeinitiatedafterthein-storagepreventivemaintenanceprogramiscomplete,andisscheduledtobeginaroundNovember1,1990.Weexpectthisphaseoftheprogramtotakeaminimumofoneyear,atwhichtimewewilldiscontinuetheinterimLIFOpolicy.

ENCLOSURE3UNRESOLVEDITEM50-244/88-23-01PAGE1OF1DEVELOPMENTANDIMPLEMENTATIONOFACOMPREHENSIVEPREVENTATIVEMAINTENANCEPROGRAMAsidentifiedinDetail6.7ofNRCInspectionReport50-244/90-09,theProcurementEngineeringGrouphasbeenassignedthetaskofdevelopment/implementationofacomprehensivepreventivemaintenanceprogramforitemsmaintainedintheGinnaStationWarehouse.Developmentofthecomputersoftwarewhichwillbeusedtoidentify,trackandcontrolitemswithpreventivemaintenancerequirementswasbegunJune15,1990withapreviouslyestimatedcompletiondateofSeptember1,1990.However,theshelf-lifeprogramisbeingdevelopedinparallelwiththepreventativemaintenanceprogram.Therefore,theestimateddateforsoftwarecompletionisnowOctober1,1990.DevelopmentoftheimplementingproceduresforthepreventativemaintenanceprogramwasbegunJuly23,1990withanestimatedcompletiondateofSeptember1,1990.However,sincedevelopmentoftheimplementingproceduresisdependentoncompletionofthesoftware,identifiedabove,theestimatedcompletiondatehaschangedtoOctober1,1990.ThisrepresentsaonemonthdeviationfromtheestimatedcompletiondateofSeptember1,1990whichwasidentifiedduringtheinspection.Basedontheimplementationdates,identifiedabove,theoverallpreventivemaintenanceprogramwillbeavailableforimplementationbyProcurementEngineeringonOctober1,1990.Atthattime,allprocureditemswillstarttobeprocessedthroughthepreventativemaintenanceprogramtoassurethatitemswithpreventativemaintenancerequirementsarecontainedinthecontrollingprogram.TheProcurementEngineeringGroupwillbeperformingareviewofpreviouslyprocureditemsmaintainedinthewarehouse.Thisprogramwillbeinitiatedafterimplementationofthepreventativemaintenanceprogram,andisscheduledtobeginNovember1,1990.Thisphaseisestimatedtohaveacompletiontimeofapproximatelyoneyear.
