Attachment to ANPP 36259 PALO VERDE NUCLEAR GENERATING STATION UNIT 1 RADIATJ.ON MONITORING UNIT NOT RESTORED TO OPERABLE WITHIN 72 HOURS Docket No. STN 50-528 License No. NPF-41 Supplement to Special Report No. 1-SR-85-032 This supplement to a Special Report is provided pursuant to ACTION 27-3 of Palo Verde Unit 1 Technical Specification Table 3.3-6 and to provide updated information regarding a Radiation Monitoring Unit not restored within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

At 2026 on Hovember 28, 1985, the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> ACTION STATEMENT for operability of the Containment High Range Monitor (RU-148) was exceeded. fr.e monitor was declared INOPERABLE at 2026 on November 25, 1985, due to a detector high voltage failure alarm. A work-authorizing document was issued to troubleshoot t the cause of the alarms and to rewcrk the radiation monitor as applicable.

Emergency Plan Inplementing Procedure #14A, which allows the use of external detectors (RU-158 B & C, RU-37 anc, 38) when the containment high range monitors (implemented 9nly in a post accident event) are INOPERABLE, may te used as a compensatory measure during inoperability of RU-148. However, no events have occurred requiring this implementation.

The root cause of this event is believed to be intermittent Iow resistance in the detector cable inside containment. The corrective action to be taken to address the root cause is repair / replacement of the detector cable. This action was originally scheduled to be completed by April 1, 1986 during the first planned outage. However, due to scheduling and parts procurement, repair has not yet been completed. The radiation monitor is expected to be returned to OPERABLE status prior to resumption of POWER OPERATION.
