At 0918 on June 26, 1986, the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> ACTION STATEMENT for operability of the Main Steam Line Radiation Monitor (RU-139) was exceeded. The monitor was declared INOPERABLE on June 23, 1986, due to spurious detector failure alarms. The initiation of a preplanned alternate program to monitor the appropriate parameter, required by Table 3 3-6 ACTION 27, was complied with ,

by the implementation of Emergency Plan Implementing Procedure EPIP-14A (Release Rate Determination).

The monitor was reset and functionally tested in an attempt to identify the cause of the alarms. The attempt to recreate the event was unsuccessful, thereby negating the ability to identify the root cause. It is known that a spurious power supply voltage anomoly will cause this response, however, this can not be confirmed as the root cause. Based upon the satisfactory results obtained during the functional test, the monitor was returned to an OPERABLE status on June 30, 1986.

Since this event has not recurred, no further corrective action is planned at this time. Should this event be repeated, appropriate actions will be taken in an attempt to establish a pattern indicative of a root cause.
