Special Report 2-SR-86-029 This Special Report is provided pursuant to Palo Verde Unit 2 Technical Specification Table 3 3-6 ACTION 27-3, and to provide information regarding a Radiation Monitoring Unit not restored to OPERABLE status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

At 0450 on August 1, 1986, the Containment Building Atmosphere Radiation Monitor (RU-1) was declared IN0PERABLE when two channels (containment atmosphere particulate radioactivity monitoring system channel and containment atmosphere gaseous radioactivity monitoring system channel) did not operate within the design specification. Efforts to return the monitor to service were unsuccessful within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> allocated under the action statement.

In accordance with Technical Specification 3.3-6, ACTION 27-2, a portable air monitor (RU-53) was immediately placed in service at 0525 as a compensatory measure. This monitor will satisfy the requirement to sample containment air for particulate and gaseous releases.

The investigation to determine the root cause was initiated under work order 167534 The root cause of this event was believed to be a defective suction pump. A new pump was installed. The post maintenance functional test further revealed that the monitor discharge solenoid valve was restricting sample flow as a result of a faulty diaphragm. The valve was rebuilt and installed per vendor specifications. The radiation monitor flow system was calibrated and RU-1 was returned to OPERABLE status on August 7, 1986 at 1642.
