We monitor was declared inoperable on January 22, 1986, as a result of the Plant Vent Low Range Radiation Monitor (RU-143) bring declared inoperable due to a failure of the counting functions.
We cause of the counting functions inoperability was the result of energizing a reactor coolant pump with the grounding straps still attached.
We resultant surge on the ground system caused a
detector and amplifier /descriminator board failure in RU-143.
We corrective action taken was the repair and calibration of RU-143, he monitoring unit was returned to service on February 10, 1986.
In addition, procedural changes are being made to prevent energizing a reactor coolant pump with the grounding straps attached.
A special report is required and made pursuant to Palo Verde Unit 2 Technical Specification Table 3.3-13 ACTION 42-b and Technical Specification 6.9.2.
