PALO VERDE NUCLEAR GENERATING STATION UNIT 2 Notification of Unusual Event of February 8,1987 Docket No. 50-529 License No. NPF-51 Special Report 2-SR-87-003 At 2144 MST on February 8,1987, the NRC Operations Center was notified via the Emergency Notification System of the declaration of a NOTIFICATION OF UNUSUAL EVENT (NUE) for the Palo Verde Nuclear Generating Station (PVNGS). The NUE was declared pursuant to Emergency Plan Implementing Procedure EPIP-02 which requires a report to the NRC for a fire which lasts longer than 10 minutes.

At 1506 on February 8,1987, with Palo Verde Unit 2 in Mode 5 (COLD SHUTDOWN) for a Surveillance Test Outage, Emergency Diesel Generator "A" was placed in operation at approximately 110% of rated capacity for 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />. At 1706, Emergency Diesel Generator "A" was reduced to 100% of rated capacity where it operated at for approximately 3i hours prior to the identification of a fire. The diesel was being tested as part of the Inte8 rated Safeguards Testing which was underway in Unit 2.

Upon receipt of fire detection alarms in the control room, an auxiliary operator was dispatched to assess the alarm. After verification of the fire detection alarms, the auxiliary operator notified the control room at 2040 that a fire was in progress at Emer8ency Diesel Generator "A". The PVNGS Fire Department was notified and responded to extinguish the fire at 2052. In accordance with EPIP-02, an NUE had been declared at 2050. The event was terminated at 2105 and the appropriate notifications were made.

Inspection of Emergency Diesel Generator "A" has revealed that the fuel oil line supplying the No. 5 cylinder (right bank) had come loose. The most extensive damage occurred in the area of the No. 4 and No. 5 cylinders. The ferrule fitting which connects the fuel. oil supply line to the fuel injector had separated thus resulting in the loose supply line. The root cause of the separated fitting is currently under investigation with the fitting manufacturer and corrective actions to prevent recurrence will be implemented based on these root cause findings. The root cause and final corrective actions will be reported in a Special Report expected to be submitted by March 9, 1987.

It has been determined that this event did not pose an actual threat to the safety of PVNGS or did not significantly hamper site personnel in the performance of duties necessary for the safe operation of PVNGS.
