i PAID VERDE NUCLEAR GENERATING STATION UNIT 2 Notification of Unusual Event of February 8,1987 Docket No. 50-529 License No. NPF-51 Special Report Supplement 2-SR-87-003-01 At 2144 MST on February 8,1987, the NRC Operations Center was notified via the Emergency Notification System of the declaration of a NOTIFICATION OF UNUSUAL EVENT (NUE) for the Palo Verde Nuclear Generating Station (PVNGS). The NUE was d:clared pursuant to Emergency Plan Implementing Procedure EPIP-02 which requires a report to the NRC for a fire which lasts longer than 10 minutes.

At 1506 on February 8,1987, with Palo Verde Unit 2 in Mode 5 (COLD SHUTDOWN) for a Surveillance Test Outage, Emergency Diesel Generator "A" was placed in operation at approximately 110% of rated capacity for 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />. At 1706, Emergency Diesel Generator "A" was reduced to 100% of rated capacity where it operated at for approximately 3A hours prior to the identification of a fire. The diesel was being tested as part of the Integrated Safeguards Testing which was underway in Unit 2.

Upon receipt of fire detection alarms in the control room, an auxiliary operator was diepatched to assess the alarm. After verification of the fire detection alarms, the auxiliary operator notified the control room at 2040 that a fire was in progress at Emergency Diesel Generator "A". The PVNGS Fire Department was notified and responded to extinguish the fire at 2052. In accordance with EPIP-02, an NUE had been declared at 2050. The event was terminated at 2105 and the appropriate notifications were made.

Inspection of Emergency Diesel Generator "A" has revealed that the fuel oil line supplying the No. 5 cylinder (right bank) had come loose. The most extensive damage occurred in the area of the No. 4 and No. 5 cylinders. The ferrule fittin8 which connects the fuel oil supply line to the fuel injection tube had disengaged thus re:ulting in the loose supply line. The root cause of the disengaged fuel injection tube has been determined to be the incorrect implementation of established manufacturing procedures for fuel injection tubes at the vendor's facility. The root cause and corrective actions have been further detailed in a report submitted pursuant to 10CFR 21 and 10CFR 50.55(e) that was provided to the NRC on Fbrch 2,1987.

It has been determined that this event did not pose an actual threat to the safety of PVNGS or did nct significantly hamper site personnel in the performance of duties necessary for the safe operation of PVNGS.
