e PALO VERDE NUCLEAR GENERATING STATION UNIT 1 Inoperable Seismic Monitoring Instrumentation License No. NPF-41 Docket No. 50-528 Special Report 1-SR-86-068 At approximately 1250 on June 13, 1986, Palo Verde Unit 1 was in Mode 1 (POWER OPERATION) at 100 percent power when the 30 day Action Statement .for operability of a seismic monitoring system was exceeded. The seismic monitoring system was declared inoperable at approximately 1205 on May 14, 1986, when a defective switch card was discovered in a seismic switch during the performance of the Seismic Monitoring System Calibration Test (36ST-1SM02).

On June 5, 1986, a replacement switch card that required repairs was obtained. Following repair, the switch card was installed on June 15, 1986, I and the Seismic Monitoring System Calibration Test was continued. During the calibration test, a defective card was discovered in the seismic monitor recorder playback unit.

i As corrective action to eturn the seismic monitoring system to operable

status, the defective card used in the seismic monitor recorder playback unit will be sent to the manufacturer for repair. The card. is projected to be returned by August 15, 1986.

Since the switch card is no longer stocked or manufactured by the manufacturer, a Design Change Package (DCP) ha been initiated to allow the switch card to be replaced by a combination trigger / switch card that is available from the manufacturer. Installation of the new trigger / switch card requires wiring changes in the seismic monitoring cabinet to allow

! installation of an additional power supply.

1 The Special Report is required and made pursuant to Palo Verde Unit 1 Technical Specification 3.3.3.3 Action Statement A.
