.c APPENDIX A General Electric Company Nuclear Energy Business Operations Docket No. 99901067/86-01 NOTICE OF NONCONFORMANCE This NRC inspection was made to follow-up on allegations involving oualification and certification of Nondestructive Examination Personnel (NDE) at General Electric (GE), Nuclear Field Services Department, Norcross, Georgia. Based on the results of this inspection conducted on November 3-7,-1986, it appears that certain of your activities were not conducted in accordance with NRC require- ,

ments. These items are set forth below and have been identified and classified in accordance with the NRC Enforcement Policy, 10 CFR Part 2, Appendix C.

1. Section 8.6.3 of GE Procedure No. QC-2, " Procedure for Qualification and Certification of Nondestructive Examination Personnel," states: "Examina-tions for Levels I and II requires a compnsite grade of 80% or greater.

In addition, each grade for the general, specific and practical shall be 70% or greater."

ContrarytotheabovealevelIINDEtechniciabwascertifiedasa Level II magnetic particle examiner (MT), and licted as so on his Certificate of Oualification, effective 1/27/84 and expiring 1/26/87, .

although he failed to attain a passing grade'of'70% on his magnetic particle examination. (86-01-01) -

2. GE Procedure No. QC-2, " Procedure for Qualification and Certification of Nondestructive Examination Personnel," states, in part, "that certification of NDE personnel is based on demonstration of satisfactory qualification by meeting the applicable requirements for education, experience, training, and attaining the required passing grades on examinations to verify physical and technical qualifications." >

i Contrary to the above, while reviewing various NDE examinations and answer sheets for compliance to proper number and types of questions as required by QC-2, threeL(3) instances were identified where examin- i ation scores were increased after full or partial credit was given for '

incorrect answers. In all three cases, credit for incorrect answers made the difference between the individuals passing or failing the examination. (86-01-02) ,
