APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Docket:- 50-313/87-02 Arkansas Nuclear One, Unit 1 License: DPR-51 During~an NRC inspection conducted during the period January 1-31,' 1987, a violation of the NRC requirements was identified. The violation involved failure to maintain a Unit 1 manual valve locked as required. In accordance with the ' General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2 Appendix C (1986), the violation is listed below:

Unit 1 Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained covering the activities recomended in Appendix ' A' of Regulatory Guide 1.33, November 1972.

Appendix A, Item C.5 requires procedures for the Emergency Core Cooling System.

Plant Operating Procedure 1102.01, " Plant Preheatup and Precritical Checklist," has been established in accordance with this Technical Specification requirement.

Attachment F, " Category E Valve Position Verification," of Plant Operating Procedure 1102.01 requires that Manual Valve MU-23 be locked open when Makeup Pump P36C is aligned for normal plant makeup service. Valve MU-23 is a discharge cross connect manual isolatica valve between Makeup Pumps P36B and P36C.

Contrary to the above, on January 26, 1987, with the plant operating at 100 percent power, and Pump P36C aligned for normal plant makeup service, the NRC inspector found that MU-23 was open as required but not locked.

The lock and chain were found on the floor below the valve.

This is a Severity Level IV Violation. (Supplement I.D) (313/83-01)

Pursuant to the provisions of 10 CFR 2.201, Arkansas Power & Light Company is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) the reason for the violation if admitted; (2) the corrective steps which have been taken and the results achieved; (3) corrective steps which'will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

Consideration may be given to extending your respcnse time for good cause shown.
