Operati.ng Licenses:
DPR-51 NPF-6 During an NRC inspection conducted April.3-7, 1989, two violations of NRC requirements were identified.
The violations involved:
(1) failure to maintain proper radiation monitor alarm setpoint and (2) radioactive liquid effluent release exceeding Technical Specification (TS) limits.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violations are listed below:
A.
Failure to Maintain Proper Radiation Monitor Alarm Setpoint TS 3.3.3.1, Table 3.3-6 requires that the alarm / trip setpoint for the ANO
. Unit 2 control room ventilation intake duct process monitor be set and maintained at a value of less than or equal to two times background during all modes of plant operations.
The TS further requires that the radiation monitoring channel alarm / trip setpoint, which exceeds the value shown in Table 3.3-6, be adjusted to within the required limit within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> or that the' monitoring channel be declared inoperable.
With the number of channels operable, less than required by the minimum channels operable requirement, within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> initiate and maintain operation of the' control room emergency ventilation system in the recirculation mode of operation.
Contrary to the above, the NRC inspector determined during a review of Licensee Event Report (LER) 368/88-14, dated October 6, 1988, that the licensee operated the ANO Unit 2 control room ventilation radiation monitor for the time period between August 11, 1988, through August 22, 1988, with an alarm / trip setpoint above the required TS limit of less than or equal to two times background beyond the allowable 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> and without placing the control room emergency ventilation system in the recirculation mode.
This violation was identified by the licensee and the NRC was notified of the noncompliance to TS requirements in the above referenced LER.
The licensee has subsequently implemented adequate corrective actions prior to this inspection to resolve the violation.
Therefore, a written response to this violation is not required.
This is a Severity Level V violation.

concentrations specified in 10 CFR Part 20, Appendix B, Table II, Column 2 for radionuclides other than dissolved or entrained noble gases.
Contrary to the above, the NRC inspector determined during a review of-LER 313/88-007, dated October 5,-1988, that the licensee made liquid ef fluent releases on September 22, 1986, and September 5, 1988, which exceeded the Maximum Permissible Concentration valves by 1.07 and 1.03, respectively.
This violation was identified by the licensee and the NRC was notified of-the noncompliance to T5' requirements in the above referenced LER.
The licensee has subsequently implemented adequate corrective actions prior to this inspection to resolve the violation. Therefore, a written response to this violation is not required.
This is a Severity Level IV violation.
