APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Docket:

50-313 Arkansas Nuclear One, Unit 1 License: DPR-51 During an NRC inspection conducted September 15 through October 23, 1988, a violation of the NRC requirements was identified. The violation involved the use of safety-related piping for a temporary support without suitable controls.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Criterion II of Appendix B to 10 CFR Part 50 and Section 2.4 of the approved Arkansas Power & Light Quality Assurance Manual - Operations (AP&L QAM), requires that activities affecting quality shall be accomplished under suitably controlled conditions.

Criterion V of Appendix B to 10 CFR Part 50 and Section 5 of the AP&L QAM requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings.

Contrary to above, the controls for temporary supports were not suitable in that on September 27, 1988, the NRC inspector found that a 6-inch safety-related pipe and a 4-inch nonsafety-related pipe had been used to support a 12-inch 'ipe undergoing modification. The safety-related piping was the "C" makeup pump suction line from the borated water storage tank.

The temporary support configuration had not been reviewed or approved by the engineering organization and were installed without documented instructions, procedures, or drawings.

This is a Severity Level IV violation.

Pursuant ta the provisions of 10 CFR 2.201, Arkansas Power & Light Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
