ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc.

50-313 Arkansas Nuclear One License No.: DPR-51 During an NRC inspection conducted on March 29 through May 9,1998, one violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:
Unit 1 Technical Specification 6.8.1 states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972.
I Paragraph A.3 of Regulatory Guide 1.33, November 1972, states, in part, that equipment control (e.g., locking and tagging) should be covered by written procedures.
l Procedure 1000.027, Revision 24, " Hold and Caution Card Control," Section 15, describes the licensee's process for the temporary removal of previously hung hold 1

cards. Step 15.2 requires an operator to document release authorization for all lead craftsman that a tagout is issued by having each lead craftsman review and sign Form 1000.027C," Partial / Temporary Release Authorization Sheet." Step 15.4 requires that a control room supervisor review the Partial / Temporary Release Authorization Sheet and the Hold Card Authorization Form and verify that all craftsmen signed in on the hold card are listed on Form 1000.027C and that release authorization has been obtained l
from all lead craftsmen.
Contrary to the above, on March 31,1998, two licensed senior reactor operators completing Form 1000.027C did not identify that two craftsmen were signed in on Hold Card 98-1-0364 and failed to obtain their authorization prior to approving the temporary release of a hold card.
This is a Severity Level IV violation (Supplement 1) (50-313/9804-01).

Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a
" Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation or severity level, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response.
