APPENDIX A NOTICE OF VIOLATION Entergy Operations, Inc.

Dockets:

50-313 50-368 Arkansas Nuclear One Licenses: DPR-51 NPF-6 During an NRC inspection conducted on September 7-24, with in-Jifice inspection of records until October 8, 1993, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Unit 1 and Unit 2 Technical Specification 6.8.1 state, in part, that " Written procedures shall be established, implemented, and maintained covering the...

refueling operations and new and spent fuel storage (e.g., Foreign Material Exclusion Areas)."

Procedure 1000.018, Revision 19, " Housekeeping," states that use of clear plastic is prohibited within the Level I housekeeping area which includes both spent fuel pools and 10 feet on all sides of both pools, and that tools, flashlights, etc., brought into the area must be tied with cord (lanyard),

either to the user's body or to a fixed object.

Procedure 1025.019, Revision 4, " Maintenance Fluid and Air System Cleanliness Control," established controls (e.g., securing badges, glasses, and pens with tape, and securing tools with lanyards) to prevent foreign material from being introduced into systems, structures or components during maintenance activities.

Contrary to the above, the following examples of failure to maintain Level I housekeeping controls were identified:

i On September 8,1993, the inspectors identified a piece of clear plastic inside the Level 1 housekeeping zone of the Unit I spent fuel pool area;

.t On September 8, 1993, the inspectors identified a screwdriver and a weight used to secure stanchions unsecured in the Level I housekeeping zone of the Unit 2 spent fuel pool area; E

On September 8, 1993, the licensee discovered a mop bucket inside the Level I housekeeping zone of the Unit I spent fuel pool area; On September 10, 1993, the inspectors identified a small wad of electrical duct tape floating in the tilt pit of the Unit 2 spent fuel pool; On September 20, 1993, the inspectors identified a piece of electrical duct tape (approximately 1-inch long) floating in the tilt pit of the Unit 1 spent fuel pool; 9311030126 931028-PDR ADOCK 05000313 G

On September 20, 1993, the licensee identified a tool (approximately 10 inches long and.1-3/4 inches in diameter) on the horizontal section of the reactor vessel former plate adjacent to core grid locations F-15 and l

G-15; On September 20, 1993, the inspectors identified two pair of anti-contamination garments inside the designated Level 1 area of the Unit I spent fuel pool; On September 22, 1993, the inspectors identified a grey paint chip (approximately 1-1/2 inch by 2-1/2 inch) floating in the Unit I spent fuel pool, and an 8-inch piece of electrical duct tape in the spent fuel pool attached to the spent fuel pool cooling pump suction strainer; and On September 28, 1993, the licensee identified an object resembling a piece of wire or nail located on the reactor core plate in grid location G-9.

These examples constitute a Severity Level IV violation (Supplement I) (313/9326-01; 368/9326-01).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations,-Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other l

action as may be proper should not be taken. Where good cause is shown,.

consideration will be given to extending the response time.
