SUBJECT: POINT BEACH INSPECTION REPORT 50-266/98021(DRP);

50-301/98021(DRP) AND NOTICE OF VIOLATION

Dear Mr. Reddemann: j

On January 4,1999, the NRC completed an inspection at your Point Beach 1 and 2 reactor

facilities. The enclosed vpection report presents the results of that inspection.

During this inspection period, Unit 2 was safely shut down for a scheduled refueling outage.

Maintenance activities were conducted in a quality manner, commensurate with the safety

significance of the work and effective in restoring safety-related equipment to service with

minimal out-of-service time. The NRC also noted conservative, risk-based decision-making j

regarding the approval and installation of a Unit 2 safety injection system modification; thereby j

reducing your plant's vulnerability to core damage from a loss of coolant accident.

Resident inspector observations during this inspection identified the need for you to continue to

focus attention on resolving material condition issues, operator workarounds, procedure

adherence issues, and configuration control. You have had and continue to have many station

program improvement initiatives to address weaknesses in these areas and your recognition of

the need for improvement is cornmendable. However, the lack of success in resolving long-

standing problems in these areas is of concern. Although no negative safety consequences

occurred, these observations indicate a need for you to focus on driving your improvement

initiatives to a successful and timely closure.

Based on the results of this inspection, the NRC has determined that a violation of NRC

requirements occurred. The violation is cited in the enclosed Notice of Violation, and the

circumstances surrounding the violation are described in the enclosed report. Although it was

licensee-identified, the violation is being cited because of its repetitive nature and .because it

was a violation that could reasonably be expected to have been prevented by corrsctive action

for a previous finding.

The NRC has concluded that information regarding the reason for the violation, and the

corrective actions taken and planned to correct the violation and prevent recurrence is already

adequately addressed on the docket in Inspection Report 50-266/98019(DRP);

50-301/98019(DRP), Licensee Event Report 50-266/98028, and the enclosed report.

Therefore, you are not required to respond to this letter unless the description therein does not

accurately reflect your corrective actions or your position. In that case, or if you choose to

provide additional information, you should follow the instructions specified in the enclosed

Notice.

that the Unit 1 minimum flow line for the safety injection pumps was frozen, rendering both

trains of the safety injection system inoperable. This issue will be reviewed further by the

resident inspectors as part of a special inspection, the results of which will be documented in

Inspection Report 50-266/99004(DRP); 50-301/99004(DRP).

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosures will be placed in the NRC Public Documer.t Room.
