Dear Sirs:

his letter will confirm the telephone report made by me to the Nuclear Regulatory Commission Duty Officer at about 12:30 pm on January 16,1991. The report concerned the complete loss of our telephone services, including both voice and burglar / supervisory alarm systems.

The loss included our four voice telephone lines, burglar and supervisory alarm systems monitored by the ADT Protective Company and the redundant burglar alann system monitored by the Narragansett Police. The failure occurred at approximately 11:30 nm, and was caused by the telephone lines being cut through while installing telephone poles.

Contact with Rhode Island Emergency Management Agency was verified using the installed two-way radio. RIEMA provided 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> monitoring of the communication link. A member of our staff remained after hours to maintain surveillance.

All telephone services were restored to normal at 9:00 am, January 18,1991. There were no consequences at the facility.
